踝部后天性真皮黑素细胞增多症1例

A Case of Acquired Dermal Melanocytosis on Ankle

患者男,61岁,无诱因左踝部蓝灰色斑4年,不疼不痒,否认有炎症、创伤、纹身及金属接触史,否认服用重金属或可能导致色素沉着的药物史。体检见左踝部蓝灰色斑,呈长方形,大小约4cm×2cm,色斑质地同正常皮肤。组织病理示表皮基底层色素增多,真皮胶原间见较多含有黑素颗粒的梭形细胞。结合临床表现、病理和免疫组织化学诊断为后天性真皮黑素细胞增多症。

A 61-year-old man presented with a 4-year history of a blue-grayish,asymptomatic oblong patch on his left ankle. The patient denied any history of preceding inflammatory diseases,trauma,tattoos or metal contact on his ankle. He also denied taking heavy metals or any medicines that may result in hyperpigmentation. Physical examination revealed an oblong gray-blue hyperpigmentation on the left ankle,which was 4 cm length and 2 cm width and had the same texture as normal skin. Histopathologic examination showed increased pigmentation in the stratum basale and a few spindleshaped cells that contained melanin among dermal collagens. The diagnosis of acquired dermal melanocytosis was made.
