miR-92a在糖尿病肾脏疾病患者血清中表达变化及与病程进展的相关性研究 被引量:9

摘要 目的探讨DKD患者血清miR-92a表达及与病程进展的相关性。方法选取T2DM患者381例,根据尿微量白蛋白/肌酐比值(UACR)分为正常白蛋白尿组(n=145)、微量白蛋白尿组(n=131)和临床蛋白尿组(n=105)。同期选取健康体检者70名作为对照组(NC)。RT-PCR检测血清中miR-92a表达。结果临床蛋白尿组、微量白蛋白尿组、正常白蛋白尿组和NC组血清中miR-92a相对表达量依次降低[(2.39±0.14)vs(1.86±0.10)vs(1.37±0.09)vs(1.00±0.03),F=3403.185,P=0.000];miR-92a相对表达量与病程、BMI、FPG、HbA_1c、FIns、HOMA-IR、TC、TG、LDL-C、Scr、SUA、BUN和UACR呈正相关(r=0.651、0.104、0.136、0.242、0.556、0.492、0.257、0.440、0.162、0.463、0.370、0.490和0.856,P<0.05),与HDL-C、eGFR呈负相关(r=-0.254、-0.787,P<0.05);多元线性回归分析显示,病程、FPG、HbA_1c、FIns、HOMA-IR、TG、HDL-C、Scr、BUN、UACR是影响患者血清miR-92a表达的独立相关因素。结论 miR-92a在T2DM患者血清中表达升高,且与DKD病程进展有关,可能参与了DKD发生发展过程。 Objective To investigate the expression changes of serum miR-92a in patients with diabetic kidney disease and its correlation with progression of disease. Methods A total of 381 patients with type 2 diabetes (T2DM) were enrolled in this study. All the patients were divided into three groups according to urinary microalbumin/creatinine ratio (UACR) : normal albuminuria group (n= 145), microalbuminuria group (n= 131) and clinical proteinuria group (n = 105). In the same period, 70 healthy people were selected as the control group(NC). The expression of serum miR-92a was detected by real-time fluorescence quantitative PCR (RT-PCR) . Results The relative expression levels of miR-92awere (2.39±0.14), (1.86±0.10), (1.37±0.09) and (1.00±0.03) in the clinical proteinuria, microalbuminuria, normal albuminuria and control group (F= 3403.185, P=0.000) . Comparison between the groups: clinical proteinuria group〉microalbuminuria group〉normal albuminuria group~〉NC group. The relative expression level of miR-92a was positively correlated with the duration of diabetes, BMI, FPG, HbAlc, Fins, HOMA-IR, TC, TG, LDL-C, Scr, SUA, BUN and UACR (r= 0. 651, 0. 104, 0. 136, 0. 242, 0. 556, 0. 492, 0. 257, 0. 440, 0. 162, 0. 463, 0. 370, 0. 490 and 0. 856,P〈0. 05), while negatively correlated with HDL-C and eGFR (r = - 0. 254 and - 0. 787, P〈 0. 05). Multiple linear regression analysis showed that the duration of diabetes, FPG, HbA1c, Fins, HOMA-IR, TG, HDL-C, Scr, BUN and OACR were independent related factors for miR-92a expression. Conclusion Serum MiR-92a expression was elevated in patients with T2DM and was associated with progression of DKD. It might be involved in the occurrence and progression of DKD.
