应用时差成像技术探讨补肾中药对高龄肾气阴两虚体外受精患者治疗结局的影响

Effects of herbs of invigorating the kidney on elder IVF patients with deficiency of both qi and yin of kidney by different time imaging technology

目的:应用时差成像技术观察补肾中药对高龄肾气阴两虚体外受精-胚胎移植(IVF-ET)患者胚胎发育时间参数的调控作用。方法:将76例行IVF-ET治疗的高龄肾气阴两虚证患者随机分为中药治疗组和安慰剂组,每组38例,另选取43例因男方因素行IVF治疗者作为正常对照组,观察各组患者肾气阴两虚症候积分变化;获卵数、2pn受精率、优胚率、临床妊娠率;时间参数及卵裂模式。结果:中药治疗组治疗后肾气阴两虚症状明显改善。中药治疗组ICSI 2PN受精率、优胚率均明显高于安慰剂组(P﹤0.05)。在t5时间段中,正常对照组及中药治疗组均明显快于安慰剂组(P﹤0.05,P﹤0.01)。结论:在IVF-ET治疗周期,应用补肾中药能改善高龄IVF-ET患者的肾气阴两虚症状,可能通过降低高龄女性早期胚胎的发育迟缓,纠正异常卵裂的发生,使高龄肾气阴两虚型IVF患者妊娠结局有所改善。

Objective： To observe the regulation effect of herbs of invigorating the kidney on embryonic development time parameter of elder in vitro fertilization-embryo transfer （IVF-ET） patients with deficiency of both qi and yin of kidney by different time imaging technology. Methods：A total of 76 IVF-ET cases with deficiency of both qi and yin of kidney were randomly divided into Chinese traditional medicine treatment group and placebo group （each had 38 cases）, and 43 cases of IVF-ET treatment caused by male factor as control group, then the syndrome score changes of deficiency of both qi and yin of kidney, the number of oocytes, the ratios of 2PN cleavage, the high-quality embryos, the clinical pregnancy, the time parameter and the cleavage patterns of each groups were observed. Results： Symptoms of deficiency of both qi and yin of kidney were significantly improved in the treatment group. Compared with the placebo group, the ratios of 2PN cleavage and high-quality embryos in ICSI in the other two groups were obviously higher then placebo group （P〈0.05）. Compared with the placebo group in t5 period, development in the other two groups were statistically faster （P〈0.05, P〈0.01）. Conclusion： During the IVF-ET treatment cycle, using herbs of invigorating the kidney could improve the symptoms of deficiency of both qi and yin of kidney in elderly patients with IVF-ET, possibly by reducing thestunting of early embryos of older women, and correcting the occurrence of abnormal cleavage, so as to improve the pregnancy outcome of the elderly IVF patients with deficiency of both qi and yin of kidney.
