血清γ-谷氨酰基转移酶水平与高血压发病风险的相关性研究 被引量:3

摘要 目的:探究血清γ-谷氨酰基转移酶(γ-glutamyltransferase,GGT)水平与高血压发病风险相关性。方法:收集符合纳入标准的研究对象246例,并对研究对象进行随访调查,共随访到221例,平均随访年限为3年。结果:48例在随访期间发生高血压,累计发病率为21. 7%。随访结果表明,随着基线血清GGT水平的升高,高血压发病率是逐渐升高的,处于第一、二、三和四分位水平各组人群的高血压发病率分别为12. 24%、20. 97%、20. 75%和31. 58%(P <0. 05)。在校正基线年龄、性别、体重指数(BMI)、糖尿病状态、吸烟、饮酒状态、体育活动、总胆固醇以及三酰甘油水平等相关混杂因素后,多元logistics回归分析结果表明,血清GGT水平处于最高分位者,与最低分位者相比,具有显著增加的高血压发病风险(OR,95%置信区间:3. 94,1. 11~13. 93)。结论:研究结果提示,血清GGT水平与高血压发病风险升高独立相关,未来仍需进一步大样本长期随访的前瞻性研究来证实此关联并揭示其内在机制。 Objective The purpose of this study was to explore the correlation between serum GGT level and the risk of hy- pertension. Method We collected participants who met the inclusion criteria from the outpatient and physical examination in our hospital. A total of 246 participants were included in the current study, we carried on the follow -up survey. A total of 221 partici- pants visited the follow- up survey. The mean follow -up period was 3 years. Results During the 3 -year follow -up,48 sub- jects developed hypertension, with a cumulative incidence of 21.7 %. The incidence of hypertension increased gradually along with the level of serum GGT at baseline. The incidence of hypertension was 12. 24% ,20. 97% ,20. 75% and 31.58% for the first, second, third and fourth quartile of serum GGT, respectively (P 〈 0. 05 ). After adjusting for age, sex, body mass index, diabetes status, smoking and drinking, physical activity, total cholesterol and triglyceride levels related confounding factors measured at baseline, the multi - variate logistic regression analysis showed that individuals with serum GGT levels in the highest quartile compared with the lowest quartile had an significantly increased risk of incident hypertension (odds ratio ,95% confidence inter- val:3.94,1.11 - 13.93). Conclusion Our findings suggest that serum GGT level is independently associated with an increased risk of hypertension. Further large - scale prospective cohort studies and experimental studies are needed to confirm this associa- tion and reveal its underlying mechanism.
