Orem自护理论结合品管圈活动在上消化道出血患者护理中的临床应用效果

Orem self-care theory combined with QCC in the clinical application effect of nursing care of patients with upper gastrointestinal bleeding

目的探讨Orem自护理论结合品管圈活动在上消化道出血患者护理中的临床应用效果。方法随机选取2016年1月～2017年12月在本院消化内科就诊的72例上消化道出血患者,采取随机数字表分配法将入选患者列入对照组和观察组,各36例。对照组患者采取品管圈持续质量改进,观察组不仅沿用对照组的品管圈护理方法,还以Orem自我护理理论指导患者提高自我护理能力。收集临床数据,观察并比较各组患者的护理效果、治疗依从性及对护理工作的满意程度。结果 (1)观察组患者护理总有效率(94.44%)显著高于对照组(75.00%),差异有统计学意义(P<0.05);(2)治疗依从性:观察组完全依从20例,部分依从13例,其余3例不依从,总依从性为91.67%,显著高于对照组(P<0.05);(3)护理满意度:观察组所有患者均对护理工作感到满意,满意度达到100%,而对照组为的护理满意度为80.56%(P<0.05)。结论 Orem自我护理模式联合品管圈活动应用于上消化道出血患者的护理效果明显,较好地提高患者治疗依从性,受到患者的欢迎,值得临床推广应用。

Objective To investigate the Orem self-care theory combined with QCC in the clinical application effect of nursing care of patients with upper gastrointestinal bleeding. Methods 72 cases of upper gastrointestinal bleeding were randomly selected from January 2016 to December 2017 in the digestive department of our hospital. The selected patients were included in the control group and the observation group by the random digital distribution method, which were all 36 cases. The control group adopted the continuous quality improvement of the product tube circle, the ob- servation group not only followed the control group＇s tube circle nursing method, but also guided the patients with the Orem self nursing theory to im- prove the self nursing ability. Collect clinical data, observe and compare the nursing effect, treatment compliance and satisfaction degree of nursing in each group. Results ①The total effective rate of nursing in the observation group （94.44%） was significantly higher than that of the control group （75.00%）, the difference was statistically significant （P〈0.05）; ②The compliance of the observation group： the observation group was fully compli- ant in 20 cases, partial compliance was 13 cases, the other 3 cases were not compliant, the total compliance was 91.67%, significantly higher than the control group （P〈0.05）; ③ Nursing satisfaction： all the patients in the observation group were satisfied with the nursing work, the satisfaction was 100%, while the control group was 80.56%（P〈0.05）. Conclusion The nursing effect of Orem self nursing mode combined with the activity of tube circle in the patients with upper gastrointestinal bleeding is obvious. It is better to improve the compliance of the patients and be welcomed by the pa- tients. It is worthy of clinical application.
