超声弹性成像及超声造影在诊断甲状腺结节中临床应用

Clinical application of ultrasound elastography and contrast-enhanced ultrasound in the diagnosis of thyroid nodules

目的探讨超声弹性成像及超声造影在诊断甲状腺结节(thyroid nodule,TN)中临床应用。方法收集本院2014年2月～2016年8月经病理确诊的TN患者146例,患者术前均行常规超声、超声造影、超声弹性成像检查,术后病理检查判断结节性质(良性或恶性),分析各检查方法特异性、灵敏性、阳性预测值、阴性预测值以及准确率。结果超声弹性成像特异性(91.85%)、灵敏性(87.95%)、阳性预测值(86.90%)、阴性预测值(92.54%)、准确率(90.37%),超声造影特异性(93.33%)、灵敏性(90.36%)、阳性预测值(89.29%)、阴性预测值(94.03%)、准确率(92.20%),均高于常规超声,且超声弹性成像联合超声造影诊断准确率达到93.12%。结论在诊断甲状腺结节中,超声弹性成像和超声造影灵敏性、特异性以及准确率较高,两者联合使用准确率更高,具有较高临床应用价值。

Objective To explore the clinical application of ultrasound elastography and contrast-enhanced ultrasound in the diagnosis of thyroid nodule （TN）. Methods 146 cases of TN patients diagnosed by pathology from February 2014 to August 2016 were collected. All patients underwent routine ultrasound, contrast-enhanced ultrasound and ultrasound elastography before operation. The pathological examination confirmed the nodule nature （benign or malignant）. The specificity, sensitivity, positive predictive value, negative predictive value, and accuracy of each test method were analyzed. Results Ultrasound elastography specificity （91.85%）, sensitivity （87.95%）, positive predictive value （86.90%）, negative predictive value （92.54%）, accuracy （90.37%）, contrast-enhanced ultrasound （93.33%）, sensitive Sex （90.36%）, positive predictive value （89.29%）, negative predic- tive value （94.03%）, and accuracy （92.20%） were higher than conventional ultrasound, and the diagnostic accuracy of ultrasound elastography com- bined with contrast-enhanced ultrasound reached 93.12%. Conclusion In the diagnosis of thyroid nodules, ultrasound elastography and contrast-en- hanced ultrasound have higher sensitivity, specificity and accuracy, and the combined use of these two is more accurate and has higher clinical appli- cation value.
