对比分析胰岛素泵与胰岛素静脉滴注治疗糖尿病酮症酸中毒的临床疗效 被引量:12

摘要 目的对比分析胰岛素泵与胰岛素静脉滴注治疗糖尿病酮症酸中毒患者的临床疗效。方法选取2015年4月~2017年6月本院收治的70例糖尿病酮症酸中毒患者作为研究对象,依据治疗方式不同分为实验组与参照组,各35例。参照组采用胰岛素静脉滴注治疗方法,实验组采用胰岛素泵治疗方法,观察比较两组患者的血酮转阴用时、尿酮转阴用时、血糖达标用时、胰岛素使用总量以及两组患者的低血糖合计率。结果实验组患者的血酮转阴用时(26.2±4.0)h、尿酮转阴用时(41.0±5.1)h与参照组的(36.0±5.3)h、(48.3±6.0)h比较差异有统计学意义(P<0.05);实验组患者的血糖达标用时(11.3±2.0)h、胰岛素使用总量(0.5±0.1)U/(kg·d)与参照组的(13.2±3.0)h、(0.9±0.3)U/(kg·d)比较差异有统计学意义(P<0.05);实验组患者的低血糖合计率2.86%与参照组的17.14%比较差异有统计学意义(P<0.05)。结论在糖尿病酮症酸中毒患者中采取胰岛素泵治疗方法对比胰岛素静脉滴注治疗方法展示更优临床疗效,具有更高应用价值。 Objective To compare the clinical eff'ect of insulin pump and insulin intravenous drip on diabetic ketoacidosis. Methods From April 2015 to June 2017, 70 patients with diabetic ketoacidosis were selected and divided into experimental group and reference group, 35 cases each. The reference group was treated with insulin intravenous drip, the experimental group was treated with insulin pump, and the two groups were observed and compared when the blood ketone mined negative, the urine ketone mined negative, and the blood glucose reached the standard. Total insulin use and hypoglycemia in both groups. Results In the experimental group, there were statistically significant differences in the time of conversion of serum ketone to negative (26.2±4.0) h, the time of conversion of urinary ketone to negative (41.0±5.1) h, and the time of conversion of urinary ketone to negative (36.0±5.3) h, (48.3±6.0) h (P〈0.05). There were statistically significant differences between the experimental group (11.3±2.0) h, the total amount of insulin (0.5~0.1) U/(kgod) and the control group (13.2±3.0) h, (0.9±0.3) U/(kgod) at the time of reaching the standard (P〈0.05). The combined rate of hypoglycemia in the experimental group was 2.86%, which was statistically significant compared with 17.14% in the control group (P〈0.05). Conclusion Insulin pump therapy is more effective in diabetic ketoacidosis than insulin intravenous drip.
