六味地黄汤加减结合胰岛素治疗妊娠期糖尿病的效果分析

Effect of six ingredients Rehmannia Decoction Combined with insulin on gestational diabetes mellitus

目的探讨六味地黄汤加减结合胰岛素治疗妊娠期糖尿病的效果。方法选取江西省宜春市人民医院2016年12月～2017年12月收治的68例妊娠期糖尿病患者为研究对象,将所有患者分为对照组与观察组各34例,对照组实施饮食、运动+胰岛素治疗,观察组实施饮食、运动+胰岛素+六味地黄汤加减治疗。比较两组患者实施治疗后临床效率、血糖控制及母婴结局情况的差异。结果观察组患者临床总有效率高于对照组(94.12%vs 70.59%),差异具有统计学意义(P<0.05);两组患者治疗前,其血糖指标比较无统计学意义;治疗后,观察组空腹血糖(3.83±0.75)mmol/L、餐后2 h血糖分别为(6.40±1.57)mmol/L,血糖水平低于对照组的(5.23±0.72)mmol/L、(8.23±2.78)mmol/L,观察组血糖控制情况优于对照组(P<0.05);观察组治疗后,胎儿窘迫、早产儿、巨大儿、新生儿窒息发生率均低于对照组,其母婴结局情况优于对照组,(P<0.05)。结论对妊娠期糖尿病患者,在饮食、运动与胰岛素治疗的基础上联合六味地黄汤加减进行治疗,可达到提高临床疗效,有效控制患者血糖水平及改善母婴结局的治疗目的。

Objective To investigate the effect of six ingredients Rehmannia Decoction Combined with insulin on gestational diabetes mellitus. Methods 68 patients with gestational diabetes mellitus admitted to Yichun People＇s Hospital of Jiangxi Province from December 2016 to December 2017 were selected as the research object. All the patients were divided into control group and observation group, 34 cases in each group. The control group was given diet, exercise ＋ insulin treatment, and the observation group was given diet, exercise ＋ insulin ＋ Liuwei Dihuang Decoction treatment. The clinical efficiency, blood glucose control and maternal and neonatal outcomes were compared between the two groups after treatment. Results The total effective rate of the observation group was higher than that of the control group （94.12% vs 70.59%）, and the difference was statistically significant （P〈0.05）.After treatment, the fasting blood glucose of the observation group was （3.83±0.75）mmol/L and the postprandial blood glucose was （6.40±1.57）mmol/L respectively. The blood glucose level of the observation group was lower than that of the control group, which was （5.23±0.72）mmol/L and （8.23±2.78）mmol/L.The blood glucose control in the observation group was better than that in the control group, （P〈0.05）; After treatment, the incidence of fetal distress, premature infants, macrosomia and neonatal asphyxia in the observation group were lower than those in the control group, and the maternal and infant outcomes were better than those in the control group （P〈0.05）. Conclusion For gestational diabetes mellitus patients, on the basis of diet, exercise and insulin therapy, combined with Liuwei Dihuang decoction can improve clinical efficacy, effectively control blood sugar levels and improve the outcome of mother and infant treatment purposes.
