卡前列素氨丁三醇联合鲜益母草胶囊在预防产后出血中的效果观察及对凝血指标的影响

An Observation of Applying Romethamine with Fresh Motherwort Capsule in the Prevention of Postpartum Hemorrhage and Its Influence on Coagulation Indexes

目的探讨卡前列素氨丁三醇联合鲜益母草胶囊在预防产后出血中的效果及对凝血指标的影响。方法选取2015年7月至2017年9月我院妇产科收治的180例具有产后出血危险因素的分娩产妇作为研究对象。观察组采用卡前列素氨丁三醇联合鲜益母草胶囊治疗,对照组采用卡前列素氨丁三醇治疗。比较两组产妇出血情况、临床疗效指标、凝血指标、血清生化指标及不良反应。结果观察组术后2 h、术后24 h及产后出血率显著低于对照组(P<0.05)。观察组的止血时间、输血率、子宫切除率均显著低于对照组(P<0.05)。观察组的FIB、D-dimer、APTT、TT均显著低于对照组(P<0.05)。观察组的血清BNP、IL-6、TNF-α水平均显著低于对照组(P <0. 05)。观察组不良反应发生率稍低于对照组(8.9%vs.16.7%),但差异无统计学意义(P>0.05)。结论卡前列素氨丁三醇联合鲜益母草胶囊可有效减少产后出血,改善凝血功能,促进产妇恢复。

Objective To investigate the effect of romethamine with fresh motherwort capsule in the prevention of postpartum hemorrhage and its influence on coagulation indexes. Methods From July, 2015 to September, 2017 in our hospital, 180 cases of delivery women with postpartum hemorrhage risk were enrolled as the research objects. The observation group was treated with romethamine and fresh motherwort capsule, while the control group was treated with romethamine. Bleeding situation, clinical curative effect index, coagulation index, serum biochemical index and adverse reaction were compared between two groups. Results The amount of hemorrhage in postpartum 2h and 24h and the rate of postpartum hemorrhage in observation group were significantly lower than the control group （P 〈 0.05 ）. The hemostasis time, blood transfusion rate and hysterectomy rate of the observation group were significantly lower than those of the control group （P 〈 0.05）. Levels of FIB, D-dimer, APTT, and TT of observation group were significantly lower than the control group （P 〈0.05）. Serum BNP, IL-6, TNF-α levels of observation group were significantly lower than the control group （P 〈 0.05 ）. The incidence of adverse reactions in the observation group was slightly lower than that in the control group （8.9% vs. 16.7% ）, but the difference was not statistically significant （P 〉 0.05 ）. Conclusion The combination of romethamine with fresh motherwort capsule can effectively reduce postpartum hemorrhage, improve coagulation function, and promote the recovery of pregnant women.
