摘要
目的探究新型蛋白myonectin与急性心肌梗死(acute myocardial infarction,AMI)患者冠状动脉病变严重程度的相关性。方法选取2017年1月至2017年7月在四川大学华西医院胸痛中心就诊,确诊为急性心肌梗死且行急诊经皮冠脉介入治疗(percutaneous coronary intervention,PCI)的80例患者作为研究对象,为观察组,另外选取性别和年龄匹配的健康志愿者69例,为对照组;测定上述对象血浆中myonectin的水平,使用Gensini积分评估急性心梗患者冠脉病变严重程度。结果急性心梗组较健康对照组血浆中myonectin水平明显降低,差异有统计学意义(χ~2=7. 23,P <0. 01)。按照急性心梗组患者冠脉病变支数分为三组,单支病变血清myonectin水平为(169. 5±44. 2)ng/mL、2支病变为(127. 9±42. 2)ng/mL和三支病变为(85.8±41.9)ng/mL,单支病变患者myonectin水平高于双支病变患者,差异具有统计学意义(χ~2=5.82,P <0.01);单支病变患者myonectin水平高于三支病变患者,差异具有统计学意义(χ~2=8.04,P <0.01);双支病变患者myonectin水平也高于三支血管病变患者,差异具有统计学意义(χ~2=4. 94,P <0. 01)。myonectin水平与Gensini积分(r=-0.790,P<0.01)、肌钙蛋白T(cTnT)(r=-0.580,P<0.01)呈负相关。结论 myonectin与急性心梗患者冠脉病变严重程度相关,可评估心梗病情。
Objective To explore the correlation between myonectin and the severity of coronary artery disease in patients with acute myocardial infarction. Methods This study included 80 patients with angiographically documented acute myocardial infarction (AMI group) and 69 healthy cases (control group). Serum levels ofmyonectinwere analyzed by ELISA, and the severity of coronary artery disease in patients with AMI was assessed using Gensini. Results The levels of myonectin in the plasma of AMI group were significantly lower than those in the control group, and the difference was statistically significant ( the difference was 7.23, P 〈 0.01 ). According to the coronary artery lesions in patients with acute myocardial infarction group count,patientswere divided into three groups: the serum level of myonectin respectively for single lesion were ( 169.5 ± 44.2 ) ng/mL, that of two lesions were ( 127.9 ± 42.2 ) ng/mL, and that ofthree lesions were (85.8 ± 41.9 )ng/mL; myonectin levelin single lesion patients was higher than that in the two branch lesions, with statistically significant difference (X2 =5.82, P 〈 0.01 ) ; that in single lesion was higher than that in three lesions, and the difference was statistically significant (X2 = 8.04, P 〈 0.01 ) ; that in two lesions washigher than that in three vascular lesions, with statistically significant difference (x2 =4.94, P 〈 0.01 ). Myonectin level was negatively correlated with Gensini ( r =- 0. 790, P 〈 0.01 ) and troponin T ( r =-0. 580, P 〈 0.01 ). Conclusion Myonectin levelisassociateted with the severity of coronary artery disease in patients with AMI.
