社区人群脑卒中高危因素及相关认知功能筛查研究

The screening study on the risk factors of stroke and related cognitive function in community

目的对农村社区进行脑卒中、认知及情绪障碍进行调查研究,为有针对性的开展脑卒中防治工作及认知障碍干预工作提供依据。方法选取2016年1月至2月江苏省常州市圩塘社区40岁及以上的常住居民(常住时间为全年经常在家或在家居住6个月以上)为研究对象,纳入2 152名居民进行脑卒中筛查,同时进行初步认知障碍及情绪状态调查。结果被调查居民中脑卒中高危者312例(14. 50%),中危人群358例(16. 64%),低危人群1 482例(68. 87%)。易引起社区居民脑卒中的常见因素中,占比例最高的为运动缺乏或轻体力劳动,尔后依次为吸烟史、高血压病、肥胖。AD8量表≥2分185例(8. 60%),MMSE提示存在可能痴呆患者157例(7. 30%)。抑郁自评量表(PHQ-9)提示可能轻度抑郁状态93例(4. 32%),可能中度抑郁状态4例(0. 19%),可能中重度抑郁状态1例(0. 05%)。广泛性焦虑量表(GAD-7)提示可能轻度焦虑状态46例(2. 14%),可能中度焦虑状态2例(0. 09%)。结论建议将运动、吸烟、降压、控制体质量等与脑卒中相关知识进行关联教育,关注卒中高危人群中存在的认知及情绪障碍。

Objective To explore the investigation and study of stroke, cognition, emotion impairment people in rural communities, and provide a basis for the targeted research on prevention and control of stroke and cognitive impairment. Methods The ubjects were currented from the pond community, changzhou, jiangsu provineedents, all above 40 years old. All 2152 subjects were screening for stroke, at the same time, a preliminary cognitive impairment and emotional state. Results There were 312 cases of high-risk residents （ 14.50% ）, 358 medium-risk cases （ 16.64% ） and 1482 low-risk residents （68.87%）. Among the common factors that cause stroke in the community, the highest proportion is the lack of exercise or light manual labor, followed by smoking history, hypertension, and obesity. AD8 seale≥ 2 points 185 （8.60 % ）, and MMSE suggested 157 cases （7.30 % ） of possible dementia. Patient health questionnaire 9 （ PHQ-9 ） indicated a mild depressive state of 93 cases （ 4. 32% ） , 4 moderate depressive state （0.19%） , and 1 moderate-severe depressive state （0.05%）. The generalized anxiety disorder 7 （ GAD-7 ） indicated a mild anxiety state of 46 cases （2.14%） and 2 cases of moderate depression status （0.09%）. Conclusion The behavior or factors such as exercise health education, smoking and hypertension.
