某精神专科医院2015～2017年辅助用药的应用分析

Application Analysis of Adjuvant Medication in a Psychiatric Hospital from 2015 to 2017

目的探讨某精神病院辅助用药的使用情况,为临床合理使用辅助用药提供参考。方法利用医院信息系统(HIS),采用回顾性分析方法,对某院2015～2017年辅助用药的品种、销售额、用药频度(DDDs)、日均费用(DDC)以及排序比(B/A)等进行统计分析。结果 2015～2017年期间总药品和辅助药品的销售金额均有所上升,辅助用药金额构成比约为35%。DDDs的总量分别为570 628、655 838和673 387。DDC的使用范围分别为1.16～25.70、1.05～25.54和1.03～23.92,DDDs排序前10位的排序比(B/A)=1的药品分别有0、1、0种;<1的药品分别有5、2、2种;>1的药品分别有5、7、8种。以肝病辅助治疗药、营养神经类和活血化瘀类为主要辅助类用药。结论该院近3年辅助用药使用集中度较好,用药结构较合理,但临床需注意个别B/A值较高的活血化瘀类药品,加强对辅助用药的干预,保障临床用药安全。

OBJECTIVE Investigate the usage of adjuvant medications in a mental hospital,so as to provide inference for proper clinical use of adjuvant medications. METHODS With the help of Hospital Information System（ HIS）,the retrospective method was used to analyze usage information namely variety,sales,Defined Daily Dose System（ DDDs）,Daily Drug Cost（ DDC） and sequence ratio（ B/A） of adjuvant medications during the period of 2015-2017 in a hospital. RESULTS During the period 2015-2017,both the whole kind of medications＇ and the adjuvant medications＇ sales amount increased while adjuvant medications＇ sales amount takes35% of the total sales amount. For each year of 2015-2017,the total amount of DDDs was 570 628,655 838 and 673 387,respectively.For each year of 2015-2017,the usage range of DDC was 1.16-25.70,1.05-25.54 and 1.03-23.92,respectively. For each year of 2015-2017 in the top 10 DDDs drug list,the number of medications with B/A values equal to 1 was 0,1 and 0,of medications with B/A values less than 1 was 5,2,and 2; of medications with B/A values more than 1 was 5,7 and 8. The main adjuvant medications were medications for liver disease,nutritional nerves,and blood circulation. CONCLUSION For the past three years the usage of adjuvant drugs in this hospital has a good degree of concentration and a reasonable structure. However,to ensure the clinical drug safety,the blood circulation and blood stasis type drugs with highly B/A values should be paid more clinical attention and the intervention should be strengthened.
