GnRH-a对子宫内膜异位症合并不孕患者保守术后妊娠结果的影响

Effect of GnRH-a on pregnancy outcome in patients with endometriosis and infertility after conservative surgery

目的探讨促性腺激素释放激素激动剂(GnRH-a)对子宫内膜异位症合并不孕患者保守术后妊娠结果的影响。方法将我院收治的80例子宫内膜异位症合并不孕患者随机分为观察组与对照组,每组40例。观察组在腹腔镜手术治疗后给予Gn RH-a治疗,对照组实施单纯腹腔镜手术治疗。比较两组围术期手术时间、出血量、下床活动时间、住院时间、妊娠率及治疗前与治疗后3、6、12个月血清糖类抗原125(CA125)水平。结果观察组手术时间、出血量、下床活动时间、住院时间与对照组比较,差异无统计学意义(P>0.05)。观察组治疗后3、6、12个月的妊娠率均高于对照组(P<0.05)。治疗后3、6、12个月,两组患者的CA125水平均低于治疗前,且观察组均低于对照组(P<0.05)。结论子宫内膜异位症合并不孕患者给予保守手术治疗后应用GnRH-a的临床疗效显著,值得临床推广。

Objective To investigate the effect of gonadotropin-releasing hormone agonist（GnRH-a） on pregnancy outcome in patients with endometriosis and infertility after conservative surgery. Methods A total of 80 cases of patients with endometriosis and infertility treated in our hospital were randomly divided into observation group and control group,with 40 cases in each group. The observation group was treated with Gn RH-a after laparoscopic surgery, and the control group was treated with simple laparoscopic surgery. The perioperative operation time, bleeding volume, ambulation time,hospitalization time, pregnancy rates and the levels of serum carbohydrate antigen 125（CA125） before treatment and at 3, 6 and 12 months after treatment in the two groups were compared. Results There were no significant differences in the operation time, bleeding volume, ambulation time, hospitalization time between the observation group and control group（P〉0.05）. The pregnancy rates in the observation group at 3, 6 and 12 months after treatment were higher than those of the control group（P〈0.05）. At 3, 6 and 12 months after treatment, the levels of CA125 in the two groups were significantly lower than those before treatment, and those in the observation group were significantly lower than the control group（P〈0.05）.Conclusion The application of Gn RH-a in patients with endometriosis and infertility after conservative surgery has satisfied clinical effect. It is worthy of clinical promotion.
