输血前感染性指标检测的临床意义

Clinical significance of pre-transfusion infectious indexes test

目的探究和分析输血前感染性指标检测的临床意义。方法从本院于2016年1月至2018年1月收治的所有进行输血的患者当中随机选取1 400例患者进行常规抽血,然后分别对乙型肝炎5项指标、丙型肝炎病毒抗体、梅毒螺旋体抗体、人类自身免疫缺陷病毒抗体等8项感染性指标进行检测及结果分析。结果阳性检出率最高的感染性指标是HBc-Ab,为26.71%,其次是HBs-Ab(15.36%)。HBs-Ab、HBc-Ab、HBe-Ab的男性感染率高于女性(P<0.05),其他指标在性别因素中显示差异不具有统计学意义(P>0.05);>60岁的患者HBe-Ab、HBc-Ab,HBe-Ag和抗-TP阳性检出率均高于≤60岁的患者(P<0.05),>60岁患者的其他感染性指标阳性检出率与≤60岁的患者比较,差异无统计学意义(P>0.05)。在1 400例患者中,有93例患者出现合并感染(6.64%)。结论对输血患者在输血前进行8项感染性指标进行检测,能够在一定程度上对医疗行为进行规范,在预防血液传播性疾病的传染、减少因输血后感染等问题所引发的各种医疗纠纷,对医院、医生、患者以及供血单位等方面具有非常重要的临床意义和价值。

Objective To explore and analyze the clinical significance of pre -transfusion infectious indexes test.Methods From January 2016 to January 2018, 1 400 patients who had received blood transfusion were randomly selectedfor routine blood draw, and five indexes of hepatitis B, hepatitis C virus antibody, syphilis antibody and humanautoimmune deficiency virus antibody were detected respectively and the results were analyzed. Results The highestpositive detection rate of infectious indexes was HBc-Ab, which was 26.71%, and the second was HBs-Ab（15.36%）. Theinfection rate of HBs-Ab、 HBc-Ab、 HBe-Ab in male were higher than those in the female（P〈0.05）, the other indicatorsshowed no statistically significant differences in gender factors （P〉0.05）. The positive detection rates of HBe -Ab,HBc-Ab, HBe-Ag and anti-TP in the patients 〉 60 years old were higher than those in the patients 〉60 years old（P〈0.05）, there were no statistically significant differences in positive detection rates of other infectious indicators of thepatients 〉60 years old and the patients ≤60 years old （P〉0.05）. Of the 1 400 patients, 93 patients had combinedinfections （6.64%）. Conclusion The pre -transfusion eight infectious indexes test for blood transfusion patients canstandardize the medical behavior to certain extent. It has great clinical significance and value to hospital, doctor, patientand blood supply unit in preventing the transmission of blood -borne diseases and reducing the number of medicaldisputes caused by post-transfusion infection.
