M2型丙酮酸激酶对肺癌合并胸腔积液的诊断价值

Investigation on the Diagnostic Value of Pyruvate Kinase M2 in Lung Cancer and Pleural Effusion

目的分析和观察M2型丙酮酸激酶(M2-PK)在肺癌合并胸腔积液当中的诊断价值。方法选取某院2013年5月-2014年8月收治的58例肺癌合并胸腔积液患者为观察对象,并将患者按照肺癌合并胸腔积液及良性胸腔积液进行分组,即A、B两组,患者分别为28例和30例;针对两组患者使用夹心ELISA法测定,记录患者M2-PK、CEA的数值,对比数值差异。结果 A组患者的M2-PK水平为(20.3±7.7)U/ml,CEA水平为(15.5±6.2)ug/L;B组患者的M2-PK水平(10.2±5.4)U/ml,CEA水平为(2.9±1.5)ug/L;I-II期患者6例,M2-PK阳性2例,CEA阳性1例;III-IV期患者24例,M2-PK阳性16例,CEA阳性13例;M2-PK诊断肺癌的灵敏度为82.14%,CEA诊断肺癌的灵敏度为71.42%。结果对比,差异存在统计学意义。结论 M2-PK在肺癌合并胸腔积液的诊断中的灵敏度、特异度高于CEA,可为临床早期发现及诊断提供有效依据。

Objective To analyze and observe the diagnostic value of M2 pyruvate kinase（M2-PK）in lung cancer with pleural effusion. Methods Fifty-eight patients with lung cancer with pleural effusion admitted from May 2013 to August 2014 in our hospital were selected and divided into two groups according to lung cancer with pleural effusion and benign pleural effusion： group A and group B,Respectively. The patients were 28 and 30 respectively. The sandwich ELISA was used to measure the M2-PK and CEA in both groups,and the differences were compared.Results The M2-PK level in group A was（20.3 ± 7.7）U/ml and the level of CEA was（15.5 ± 6.2）ug/L in group A; the level of M2-PK（2.9 ± 1.5）ug/L. There were 6 cases of stage I-II,2 cases of M2-PK positive,1 case of CEA positive,24 cases of stage III-IV,16 cases of M2-PK positive and 13 cases of CEA positive. The sensitivity of M2-PK in diagnosing lung cancer was 82.14%,and the sensitivity of CEA in diagnosing lung cancer was 71.42%. A contrast on the results showed that there was a statistically significant difference. Conclusions The sensitivity and specificity of M2-PK in the diagnosis of lung cancer with pleural effusion were higher than that of CEA,which might provide an effective basis for the early detection and diagnosis of M2-PK.
