社区慢性病管理中群组管理的应用价值

The Application Value of Group Management in Community Chronic Disease Management

目的观察社区慢性病管理中群组管理的应用价值。方法随机选取3个社区2016年6月至2017年6月接受社区医疗管理的128例慢性病患者为本次实验研究对象,按照是否给予群组管理将所有患者均分为实验组与对照组,对照组64例慢性病患者给予常规管理模式,实验组64例患者给予群组管理模式,观察两组慢性病患者临床干预效果。结果实验组患者饮食知识、规律用药、定期复诊、运动锻炼等相关内容达标率面明显优于对照组,两组患者各项数据差异具有统计学意义(P<0.05)。结论对于社区慢性病患者给予群组管理可有效改善个体血压、血糖等相关指标,提高个体患者对自身疾病管理能力。

Objective To observe the application value of group management in community chronic disease management. Methods 128 chronic patients receiving community medical management in 3 communities from June 2016 to June 2017 were selected as the experimental subjects. According to the group management, all the patients were divided into the experimental group and the control group, 64 cases of chronic diseases in the control group were given routine management mode, and 64 patients in the experimental group were given the group management mode to observe the clinical intervention effect of two groups of patients with chronic diseases. Results The diet knowledge, regular medication, regular treatment, exercise and other related contents of the experimental group were obviously superior to those of the control group. The differences in the data of the two groups were statistically significant（P〈0.O5）. Conclusion The group management can effectively improve the blood pressure, blood sugar and other related indicators for the patients with chronic diseases in the community, and can also improve the ability of individual patients to manage their own diseases.
