Narcotrend监测下精确全麻联合区域阻滞老年人术后认知功能障碍的影响

The Effects of Accurate General Anesthesia Combined with Regional Block on Postoperative Cognitive Dysfunction in Elderly Patients Monitored by Narcotrend

目的研究观察Narcotrend监测下精确全麻联合区域阻滞老年人术后认知功能障碍的影响。方法选取我院自2017年4月至2018年4月收治的60例全麻下进行手术的患者作为研究对象,并随机分为对照组(n=30)与实验组(n=30),对照组根据麻醉师临床经验判断麻醉深度并用药,实验组以NT监测数据判断麻醉深度并用药,对比两组认知功能情况。结果实验组术中简易智力状态检查法(MMSE)评分高于对照组,两组间对比差异有统计学意义(P<0.05);但两组患者术前及术后MMSE评分差异不大,无统计学意义(P>0.05)。结论对老年患者手术麻醉中行Narcotrend监测,能精准确定麻醉深度,减少患者术后认知功能障碍的发生,值得在临床中应用。

Objective To study the effects of accurate general anesthesia combined with regional blockade on postoperative cognitive dysfunction in elderly patients monitored by Narcotrend. Methods 60 patients underwent surgery under general anesthesia admitted to our hospital from April 2017 to April 2018 were selected as subjects and randomly divided into control group（n =30） and experimental group（n =30）. The control group judged the depth of anesthesia according to the clinical experience of the anesthesiologist and used the drug. The experimental group used the NT monitoring data to judge the depth of anesthesia and used the drug to compare the cognitive function of the two groups. Results The scores of the simple intelligence state examination（MMSE） in the experimental group were higher than those in the control group. The difference between the two groups was statistically significant（P〈0.05）. However, the difference between the preoperative and postoperative MMSE scores was not large in the two groups, no statistical significance（P〉0.05）. Conclusion Narcotrend monitoring in elderly patients with surgical anesthesia can accurately determine the depth of anesthesia and reduce the occurrence of postoperative cognitive dysfunction. It is worthy of clinical application.
