糖尿病性视网膜病变患者住院费用及其影响因素分析

Analysis on the Hospitalization Expenses of In-patients with Diabetic retinopathy and Its Affected Factors

目的分析糖尿病性视网膜病变患者的住院费用构成及其影响因素,了解患者与家庭的疾病经济负担。方法收集2015年-2016年黑龙江省某三甲医院的糖尿病性视网膜病变患者住院信息共982例,单因素分析采用非参数Kruskal-Wallis秩和检验方法,分析不同因素组内部的费用是否有显著差异,多因素分析采用半对数线性回归模型对费用的影响因素进行分析。结果糖尿病性视网膜病变患者的次均住院费用为9889元,住院费用构成中的前两位是药品费用和诊断类项目费用,分别占总费用的38.40%和24.49%;治疗方式和住院天数是影响患者住院费用的主要因素。结论糖尿病性视网膜病变的住院费用给患者带来了较大的经济负担,明确诊断标准并严格执行临床路径,合理降低住院天数是控制住院费用的有效途径。

Objectives To analyze the composition of hospitalization expenses and its influencing factors of diabetic retinopathy and explore the economic burden of this disease. Methods A total of 982 patients with diabetic retinopathy in a tertiary hospital in Heilongjiang province were collected in 2015 and 2016,and the nonparametric test was used to determine whether there were significant differences in the cost of the different variables. And the factors influencing the cost were analyzed by using the semiparametric linear regression model. Results The expenditure of hospitalization with diabetic retinopathy was 9889 yuan,which was mainly composed by medicine expenditure 38.4% and diagnostic items expenditure 24.5%.The main factors to the hospitalization expenses were：treatment,hospitalization days and medical insurance. Conclusions The effective ways to control the hospitalization expenses of in-patients with diabetic retinopathy were shorting hospitalization days,controlling drug costs and standardizing diagnosis.
