超薄股前外侧穿支皮瓣与传统皮瓣的比较研究

A comparative study of anterior lateral perforated flap with traditional flap

目的：探讨应用游离超薄股前外侧穿支皮瓣与传统皮瓣移植修复手足创面的比较效果研究。方法：随机抽取我院2014年1月-2016年1月间收治的手足创面损伤患者44例为研究对象，随机分为对照组和观察组，对照组采用传统皮瓣移植修复手术，观察组采用游离超薄股前外侧穿支皮瓣移植修复手术。观察2组患者皮瓣是否全部存活，观察皮瓣的血运、肤色和外观。比较2纽患者的治疗效果和治疗前后运动功能和感觉功能评分的变化。结果：比较2纽患者移植修复手术的临床治疗效果，观察组好于对照组（P〈0．05）；比较2组患者治疗前后手足DASH评分和感觉功能单丝法评分，治疗前无明显差别（P〉0．05），治疗后观察组好于对照组（P〈0．05）。结论：对于治疗手足软组织缺损，应用超薄股前外侧穿支皮瓣游离移植修复比传统皮瓣移植修复治疗效果较好，不仅治疗时间短，愈合时间快，而且皮瓣移植全部存活，创面皮肤质地柔软，肤色红润，外观良好，值得在临床治疗上推广使用。

Objective： To investigate the comparative effect of applying free ultrathin femoral anterior lateral perforated flap and tradi- tional flap grafting to repair limbs. Methods： randomly from January 2014 to January 2016 were of 44 patients with hand, foot and wound injury as the research object, were randomly divided into control group and research group, control group with traditional flap transplanta- tion surgery to repair, the team using the free thin strands of anterolateral perforators flap transplantation surgery to repair. To observe whether the skin flap of the two groups all survived, observe the blood flow, skin color and appearance of the flap. Compare the therapeu- tic effect of the two groups and the changes in the function of motor function and sensory function before and after treatment. Results：compared with the control group （ P 〈 0.05）, the results were compared with those of the two groups. Compared to the two groups, there was no significant difference （ P BBB 0 0.05 ） before and after treatment, and the study group was better than the control group （ P 〈 0. 05 ）. Conclusion： for treatment of hand, foot and soft tissue defect, thin strands of free transplantation of anterolateral perforators flap to repair than traditional repair skin flap transplantation treatment effect is good, not only the treatment time was short, fast healing time, and all flap transplantation survival, wound skin texture was soft, color of skin ruddy, appearance was good, should be worth popularizing in clinical treatment.
