超声造影早期诊断肝硬化合并小肝癌的价值观察

Significance of ultrasonography early diagnosis in 120 patients with liver cirrhosis complicated with small liver cancer

目的探讨肝硬化合并小肝癌患者采用超声造影的早期诊断价值。方法选取2016年6月至2017年12月间西京医院收治的120例肝硬化合并小肝癌患者,均于早期行超声造影检查,比较病理结果,分析超声造影诊断价值。结果经病理结果确诊的120例肝硬化合并小肝癌患者行超声造影检查确诊114例,准确率为95.0%;漏诊6例,漏诊率为5.0%。中-低分化癌与高分化癌超声造影开始增强时间与增强峰值时间比较,差异无统计学意义(P>0.05);中-低分化癌超声造影开始消退时间短于高分化癌,差异有统计学意义(P<0.05)。超声造影病灶检出率为94.4%,明显高于常规CT的69.3%与增强CT的77.2%,差异均有统计学意义(均P<0.05)。结论肝硬化合并小肝癌患者采用超声造影,早期诊断率较高,值得临床应用。

Objective To analyze the early diagnostic value of ultrasonography in patients with liv- er cirrhosis complicated with small liver cancer. Methods A total of 120 patients with liver cirrhosis com- plicated with liver cancer treated at Xijing Hospital were selected from June 2016 to December 2017. They all underwent early ultrasonic diagnosis. The pathological results were compared and the diagnostic value of contrast-enhanced ultrasound was analyzed. Results For the 120 patients with liver cirrhosis complicated with small liver cancer, 114 patiehts who had ultrasound contrast examination were confirmed by pathologi- cal results and, the accuracy rate was 95.0%. Six patients were omitted, and the omission diagnose rate was 5.0%. There was no significant difference in initial enhancement time and the enhancement peak time be- tween the medium to low differentiated carcinoma and the high differentiated carcinoma （ P 〉 0.05 ）. The recession time of contrast-enhanced ultrasound in medium to low differentiated carcinoma was significantly shorter than in highly differentiated carcinoma （ P 〈 0. 05 ）. The detection rate of lesions in contrast en- hanced ultrasound was 94. 4%, which was significantly higher than 69. 3% of conventional CT and 77. 2% of the enhanced CT （ all P 〈 0. 05 ）. Conclusion The early diagnosis rate of nltrasonography in patients with liver cirrhosis complicated with small liver cancer is high, which is worthy of clinical application.
