青少年白血病患者治疗期的生活质量及其影响因素

Quality of life and its influencing factors in the treatment of adolescent patients with leukemia

目的探讨未成年和大学生白血病患者治疗期间的生活质量情况及其影响因素。方法选取2015年1月至2018年1月间安徽省立医院血液科收治的77例未成年白血病患儿为未成年组和77例大学生白血病患者为大学生组,统计两组患者治疗期间生活质量评分,开展单因素和Logistic多因素回归分析生活质量的影响因素。结果未成年组患儿生活质量分析,年龄、是否初治、家庭经济状况、医护满意度和KPS评分,差异均有统计学意义(均P<0.05)。大学生组患者生活质量分析,是否初治、家庭经济状况、医护满意度、KPS评分和治疗依从性,差异均有统计学意义(均P<0.05)。多因素分析,年龄小、KPS评分低、家庭状况不佳和医护满意度差是影响未成年组患儿生活质量的危险因素。治疗依从性不佳、KPS评分低和医护满意度差是影响大学生组患者生活质量的危险因素。结论 KPS评分低和医护满意度差是未成年和大学生白血病患者治疗期间影响生活质量的共同危险因素。

Objective To investigate the quality of life and its influencing factors during the treat- ment of adolescent patients with leukemia. Methods A total of 77 adolescent patients with leukemia were enrolled in the juvenile group and 77 college students with leukemia were enrolled in the college students group who were treated at Department of Hematology, Anhui Provincial Hospital from January 2015 to Janu- ary 2018. The quality of life scores during the treatment period were counted. Univariate and logistic multi- variate regression analysis were used to investigate the factors influencing quality of life. Results In the a- nalysis of quality of life in the juvenile group, the differences in terms of age, initial treatment, family eco- nomic status, medical satisfaction, and Karnofsky performance score （KPS） were statistically significant （ all P 〈 0. 05 ）. For the college students, the differences in terms of initial treatment, family economic sta- tus, medical satisfaction, KPS and treatment compliance were statistically significant （ all P 〈 0. 05 ）. Ac- cording to the results of multivariate analysis, young age, low KPS, poor family status, and poor medical satisfaction are risk factors for the quality of life in adolescent leukemia patients. Poor treatment adherence, low KPS, and poor medical satisfaction are risk factors affecting the quality of life in college student patients with leukemia Conclusion The low KPS and poor medical satisfaction are the common risk factors for the quality of life in both adolescent leukemia patients and college student patients with leukemia.
