游离腓肠内侧动脉穿支皮瓣修复虎口软组织缺损

Free medial sural artery perforator flap to repair soft tissue defect of the first web

目的探讨游离腓肠内侧动脉穿支皮瓣修复虎口创面的临床疗效。方法 2014年8月-2017年3月,应用游离同侧腓肠内侧动脉穿支皮瓣修复虎口区创面13例,均Ⅱ期修复,缺损面积:4.0 cm×5.0 cm^5.0 cm×8.0 cm,伴有不同程度的肌腱及骨外露,皮瓣供区直接缝合。结果术后13例皮瓣成活并随访,随访时间6~24个月,平均11个月,皮瓣外形、质地及外展功能良好。两点辨别觉为1.8~3.2mm。结论游离腓肠内侧动脉穿支皮瓣修复虎口创面质地良好,血供可靠,临床疗效满意,值得推广。

Objective To investigate the clinical effect of free medial sural artery perforator flap on the repairing of the wound of the first web. Methods From Aug. 2014 to Mar. 2017, the medial sural artery perforator flap was used to repair 13 cases of wound in the first web area. All the wounds were repaired in two stages. The defect area was 4.0 cm×5.0 cm to 5.0 cm×8.0 cm, accompanied by different degrees of tendon and bone exposure, and the donor site was sutured directly. Results All the flaps survived in 13 cases. All patients were followed up for 6-24 months, with an average of 11 months. The abductor function of the thumb recovered well. The appearance and texture of the flap were good, and the two- point discrimination of skin flap was 1.8 to 3.2 cm. Conclusion The free medial sural artery perforator flap has the advantages of good texture, rehable blood supply and satisfactory chnical curative effect, it is worthy of popularization.
