闭合复位后路空心螺钉对锁骨中段骨折患者术后肩关节功能的影响

Effect of Closed Reduction with Posterior Hollow Screw on Shoulder Joint Function in Patients with Middle Clavicle Fracture

摘要 目的观察闭合复位后路空心螺钉内固定对锁骨中段骨折患者术后骨折愈合时间及肩关节功能的影响。方法选取2016年5月-2017年6月焦作市人民医院锁骨中段骨折患者78例,采用随机数表法分组,各39例。对照组行切开复位内固定术,观察组行闭合复位后路空心螺钉内固定术。对比两组手术相关情况(手术时长、术中出血量、骨折愈合时间)及并发症发生情况,采用肩关节Neer及Constant-Murley评分评价对比术后两组肩关节功能。结果观察组手术时长、骨折愈合时间较对照组短,术中出血量较对照组少(P<0.05);术后观察组肩关节Neer评分、Constant-Murley评分均较对照组高(P<0.05)。结论闭合复位后路空心螺钉内固定治疗锁骨中段骨折患者,可减少术中出血,缩短骨折愈合时间,促进患者肩关节功能恢复,安全性高。 Objective To explore the effect of closed reduction with posterior hollow screw on fracture healing time after operation and shoulder joint function in patients with middle clavicle fracture. Methods 78 patients with clavicle fracture in Jiaozuo People's Hospital from May 2016 to June 2017 were randomly divided into control group and observation group,39 cases in each group.The control group was treated with open reduction and internal fixation,and the observation group underwent closed reduction and posterior cannulated screw internal fixation. The operation related conditions( duration of operation,intraoperative blood loss,fracture healing time) and complications were compared between the two groups. The shoulder joint Neer and Constant-Murley scores were used to compare the shoulder functions of the two groups after operation.Results In the observation group,the operative time and fracture healing time were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,and the difference was statistically significant( P0. 05).The shoulder joint Neer score and Constant-Murley scores of the observation group were higher than those of the control group,and the difference was statistically significant( P0.05).Conclusion The closed reduction and posterior hollow screw fixation in the treatment of middle clavicular fracture can reduce intraoperative bleeding,shorten the time of fracture healing,and promote the recovery of shoulder joint function.It has high safety.
