视频教学联合授权理论在创伤性颞下颌关节强直患者张口功能训练中的应用效果 被引量:9

Effects of video teaching combined with empowerment theory in open mouth functional training among traumatic temporomandibular joint ankylosis patients

摘要 目的探讨视频教学联合授权理论在创伤性颞下颌关节强直患者张口功能训练中的应用效果。方法选取2015年4月-2017年4月于郑州大学第一附属医院口腔科住院治疗的创伤性颞下颌关节强直患者60例为研究对象,按奇偶数分为对照组和观察组,每组各30例。对照组采用视频教学模式,观察组在对照组的基础上结合授权理论进行口腔功能训练。采用患者积极度量表(PAM)、疼痛数字评分法(NRS)评估两组患者的积极度和疼痛程度,比较两组患者干预前后张口受限程度及最大张口度。结果干预后6个月,观察组患者PAM评分高于对照组,NRS评分低于对照组,差异有统计学意义(P〈0.05)。干预后6个月,两组患者张口受限程度差异无统计学意义(P〉0.05),观察组患者最大张口度大于对照组患者,差异有统计学意义(P〈0.01)。结论视频教学联合授权理论可提高创伤性颞下颌关节强直患者的积极度和最大张口度,是减轻患者疼痛感的有效方法。 Objective To explore the effects of video teaching combined with empowerment theory in functional training of open mouth among traumatic temporomandibular joint ankylosis patients. Methods From April 2015 to April 2017, we selected 60 traumatic temporomandibular joint ankylosis inpatients of Department of Stomatology in the First Affiliated Hospital of Zhengzhou University as subjects. All of the patients were divided into control group and observation group according to odd or even, 30 cases in each group. Patients of control group received the mode of video teaching. On this basis of control group, observation group combined with empowerment theory to provide functional training of open mouth for patients. We evaluated the positivity and pain with the Patient Activation Measure (PAM) and Numerical Rating Scales (NRS) and compared the limitation of open mouth as well as the maximum extent of open mouth before and after intervention among patients in two groups. Results Six months after intervention, the score of PAM of patients in observation group was higher than that in control group; the score of NRS was lower than that in control group ; the differences were significant (P 〈 0.05). Six months after intervention, there was no statistical difference in the limitation of open mouth among patients in two groups (P 〉 0.05). The maximum extent of the open mouth of patients in observation group was greater than that in control group with a significant difference (P 〈 0.01). Conclusions Video teaching combined with empowerment theory can improve positivity and maximum extent of the open mouth of traumatic temporomandibular joint ankylosis patients. It is an effective way to reduce patients ' pain.
