急性心肌梗死患者中心肌肌钙蛋白与心肌酶谱的诊断价值对比

Comparison of Diagnostic Value of Cardiac Troponin and Myocardial Enzymes in Patients with Acute Myocardial Infarction

目的对比分析急性心肌梗死患者中心肌肌钙蛋白与心肌酶谱的诊断价值。方法选取于2015年6月至2017年12月本院收治的420例急性心肌梗死患者进行研究,将其设置为研究组,同期选取到本院体检的健康者420例为对照组。两组均将予以心肌肌钙蛋白、心肌酶谱进行诊断。对比两组检查结果,并比较心肌肌钙蛋白、心肌酶谱的具体诊断价值。结果研究组患者的心肌酶谱结果:LDH(85.95%)、CK(71.90%)、CK-MB(72.86%);c Tn I阳性率为100%。对照组健康体检者的心肌酶谱阳性率和c Tn I阳性率均为0%。组间阳性率比较,差异有统计学意义(P<0.05)。结论对急性心肌梗死患者测定心肌酶谱,可获得较高的诊断价值,但其特异度、敏感度均不及心肌肌钙蛋白,故心肌肌钙蛋白测定的诊断价值更高,值得大力推广。

Objective To compare the diagnostic value of cardiac troponin and myocardial enzymes in patients with acute myocardial infarction. Methods A total of 420 patients with acute myocardial infarction admitted to our hospital from June 2015 to December 2017 were enrolled in the study group. 420 healthy subjects who were selected for physical examination in the same period were selected as the control group. Cardiac troponin and myocardial enzymes were diagnosed in both groups. The results of the two groups were compared and the specific diagnostic value of cardiac troponin and myocardial enzymes was compared. Results Myocardial zymogram results of the study group： LDH（85.95%）, CK（71.90%）, CK-MB（72.86%）; c Tn I positive rate was 100%. The positive rate of myocardial enzymes and the positive rate of c Tn I in the healthy subjects of the control group were both 0%. The positive rate between the groups was statistically significant（P〈 0.05）. Conclusion The myocardial enzyme spectrum of patients with acute myocardial infarction can obtain high diagnostic value,the diagnostic value of cardiac troponin determination is higher, which is worthy of promotion.
