脐血皮质醇测定对评价新生儿缺氧缺血性脑病的临床价值

Clinical Value of Umbilical Cord Blood Cortisol in Evaluation of Neonatal Hypoxic-ischemic Encephalopathy

目的探讨脐血皮质醇水平测定与新生儿缺氧缺血性脑病的相关性,为预先干预提供依据。方法随机选取我院自2015年2月至2018年1月足月缺氧缺血性脑病婴儿82例作为观察组,并选取同期出生的健康婴儿82例作为对照组,对新生儿脐血血清COR水平进行比较;在出生后第14天对所有婴儿进行20项行为神经测定评分,将所测到的血清COR数据、新生儿行为神经测定评分结果进行相关性分析。结果观察组婴儿的血清COR水平均高于对照组,且随着婴儿临床分级的加重而升高;出生后第14天的行为神经测定评分,观察组的婴儿平均得分低于对照组。结论脐血COR水平的高低,对于预测新生儿是否会发生缺氧缺血性脑病具有较可靠的参考性,可以作为临床上对缺氧缺血性脑病早期诊断和病情评估的参考指标,而且脐血COR水平与缺氧缺血性脑病之间具有较好的相关性,能够提供诊断指导意义,同时能够为近期预后提供参考。

Objective To explore the correlation between the measurement of cord blood cortisol level and neonatal hypoxic ischemic encephalopathy, so as to provide a basis for preintervention. Methods 82 infants with hypoxic ischemic encephalopathy from February 2015 to January 2018 in our hospital were random Ly selected as the observation group, and 82 healthy infants born at the same time were selected as the control group to compare the COR level of neonatal umbilical blood serum. All infants were given 20 behavioral neurologic test scores on the 14 th day after birth, and the correlation analysis of serum COR data and neonatal behavioral neurologic scores was conducted. Results The serum COR level of infants in the observation group was higher than that of the control group. On the 14 th day after birth, the scores of behavioral neuropathy were lower in the observation group than in the control group. Conclusion Umbilical cord blood level of COR, whether for predicting neonatal hypoxic ischemic encephalopathy will happen with a reliable reference, can be used as a clinical evaluation of hypoxia ischemic encephalopathy, early diagnosis and a reference indicators, and umbilical cord blood COR level and has a good correlation between hypoxia ischemic encephalopathy, can provide diagnostic significance, at the same time can provide a reference for the recent prognosis.
