妊娠期妇女HbAlc水平测定及其在妊娠期糖尿病的早期筛查及诊断应用 被引量:2

摘要 目的探讨研究妊娠期妇女不同阶段的糖化血红蛋白水平,分析糖化血红蛋白水平对于妊娠期糖尿病患者的早期诊断作用。方法本次研究对象为2017年1月至2018年4月在我院进行检查的妊娠期妇女和妊娠期糖尿病患者,选取妊娠期妇女、妊娠期糖尿病患者和正常妇女各20名,分为正常妇女组、妊娠期妇女组和妊娠期糖尿病妇女组,检测三组研究对象的糖化血红蛋白水平,对研究对象空腹、服用葡萄糖后2 h的血糖水平进行检测并统计结果。结果妊娠期妇女组晚期的糖化血红蛋白指标明显高于早期和中期的糖化血红蛋白水平,P<0.05,但与正常女性的糖化血红蛋白指标差异不大。若将妊娠期糖尿病的临界点标为5.8%的话,糖化血红蛋白在妊娠期糖尿病的早期筛查中的敏感性和特异性分别为90.1%和73.1%。结论妊娠期妇女在不同阶段的糖化血红蛋白水平会发生变化,晚期会高于早期和中期,且糖化血红蛋白在妊娠期糖尿病早期筛查中有较高的诊断价值,值得临床广泛应用。 Objective to explore and study glycated hemoglobin level of pregnant women at different stages, and analyze early diagnostic effect of glycated hemoglobin(HbA1c) on gestational diabetes. Methods treat pregnant women and gestational diabetes patients examined in our hospital from January 2017 to April 2018 as study objects, choose 20 cases pregnant women, gestational diabetes and normal women each, divide them into normal group, pregnancy group and gestational diabetes group. Detect HbA1c level, fasting blood glucose level and that 2 hours after taking glucose of three groups, carry on statistics of effect. Results late glycosylated hemoglobin index of pregnancy group was significantly higher than early and middle period, P〈0.05, which had little difference from normal group. With gestational diabetes critical point of 5.8%, early screening sensitivity and specificity of glycated hemoglobin of gestational diabetes was 90.1% and 73.1% respectively. Conclusion glycosylated hemoglobin level of pregnancy women may change at different stages, that of late period may be higher than early and middle period, and glycated hemoglobin has high diagnostic value in early screening of gestational diabetes, which is worthy of wide clinical application.
