全科医生激励支付制度对糖尿病患者健康的影响

Impact of general practitioner incentive payment system on health of diabetic patients

目的探讨全科医生激励支付制度对2型糖尿病患者健康结果的影响。方法本研究随机选择南山区人民医院荔湾社区健康服务中心收治的2型糖尿病患者200例,分为激励组和对照组各100例。对照组医生采用现有支付方案,2型糖尿病管理采用现有管理方案,激励组医生采用激励支付方案,2型糖尿病管理实施统一的健康干预方案。比较两组患者的健康结果。结果(1)激励组2型糖尿病患者完全遵医用药、遵医监测血糖和通过改变生活方式干预血糖的行为均优于对照组,差异有统计学意义(P<0.05);(2)激励组患者的血脂、血糖水平均优于对照组,差异有统计学意义(P<0.05)。结论全科医生激励支付制度能有效改善2型糖尿病患者的健康指标,值得临床推广。

Objective To analyze the impact of general practitioner incentive payment system on health outcomes in patients with type 2 diabetes mellitus. Methods 200 patients with type 2 diabetes mellitus from Liwan Community Health Service Center of Shenzhen NanShan District People＇s Hospital were randomly selected and divided into incentive group and control group, 100 cases in each group. The con trol group used the existing payment scheme, and the management of type 2 diabetes adopted the existing management plan. The incentive group used incentive payment scheme, and the management of type 2 diabetes adopted a unified health intervention program. The health outcomes of the two groups were compared. Results（ 1） The behaviors of complete compliance with medicines, monitoring blood glucose and intervening blood glucose by changing lifestyle in the incentive group were better than those in the control group, the differences were statistically significant（P〈0.05）.（2） The blood lipid and blood glucose of the patients in the incentive group were better than those in the control group, the differences were statistically significant（P〈0.05）. Conclusion Incentive payment system for general practitioners can effectively improve the health indicators of type 2 diabetes mellitus and is worthy of clinical promotion.
