基于助产士主导的护理服务对初产妇妊娠结局的影响

Influence of midwife-oriented nursing service on pregnancy outcome of the primiparas

目的探讨基于助产士主导的护理服务对初产妇妊娠结局的影响。方法回顾性分析2017年7～9月在我院接受传统助产服务模式的初产妇616例临床资料为对照组,并选取2017年10～12月在我院给予助产士主导的护理服务的初产妇615例临床资料为观察组,比较两组简化中文版分娩自我效能量表评分、产程时间及妊娠结局情况。结果观察组孕34周时CBSEIC-32评分与对照组比较,差异无统计学意义(P> 0.05);观察组孕38周时CBSEIC-32评分明显高于对照组,差异有统计学意义(P <0.05);观察组第一产程、第二产程及总产程时间均短于对照组,差异有统计学意义(P <0.05);观察组会阴侧切、新生儿窒息及产后出血发生率均低于对照组,差异有统计学意义(P <0.05)。结论给予助产士主导的护理服务可有效提高初产妇分娩自我效能,缩短产程时间,改善妊娠结局。

Objective To investigate the influence of midwife-oriented nursing service on the pregnancy outcome of the primiparas. Methods The clinical data of 616 primiparas who admitted into our hospital from July 2017 to September 2017 were retrospectively analyzed, and they were taken as the control group, and these pregnant women were given traditional birth attendance model; the clinical data of 615 primiparas who admitted into our hospital from October 2017 to December 2017 were retrospectively analyzed, and they were taken as the observation group, and they were given midwife-oriented nursing service. The short form of the Chinese childbirth self-efficacy inventory（CBSEIC-32） score, birth process time, pregnancy outcome were compared between the patients in the two groups. Results The CBSEIC-32 scores of the observation group and the control group at 34 gestational weeks were compared, and there was no significantly statistical differenee（P 〉 0.05）; The CBSEIC-32 score at 38 gestational weeks in the observation group was significantly higher than that in the control group, and there was statistical differenee（P 〈 0.05）; The time of first stage of labor, time of second stage of labor and time of total stage of labor in the observation group were significantly shorter than those in the control group, and there was statistical difference（P 〈 0.05）; The incidence of episiotomy, neonatal asphyxia, postpartum hemorrhage in the observation group were significantly lower than those in the control group, and there was statistical differenee（P 〈 0.05）. Conclusion The application of midwife-oriented nursing service for the primiparas is able to effectively increase the self-efficacy, shorten the birth process time and improve the pregnancy outcome.
