婴幼儿抗生素相关性腹泻的危险因素分析及护理

Infantile Diarrhea Associated with Antibiotics:Risk Factor Analysis and Nursing

为探讨婴幼儿抗生素相关性腹泻(AAD)的危险因素,以期为临床护理工作提供指导,本研究回顾我院儿科收治的应用抗生素治疗的399例患儿资料,根据是否发生AAD分组,分析影响AAD发生的危险因素。结果显示,本组发生AAD 91例(22.8%)。单因素分析显示,AAD的发生与患儿年龄、疾病严重程度、血红蛋白水平、抗生素应用时间、联合用药种类、激素使用、微生态制剂使用、侵袭性操作、住院时间相关,P <0.05。Logistic回归分析显示,病情重、抗生素应用时间长、联合用药种类≥2种、使用微生态制剂、侵袭性操作是AAD的独立相关因素,P <0.05;使用微生态制剂是AAD的保护因素(OR<1)。结果表明,婴幼儿AAD发生率高,病情重、抗生素应用时间长、多药联合应用、侵袭性操作是AAD的危险因素,临床应合理应用抗生素,预防性应用微生态制剂防止AAD的发生。

To inv direction for clini ceived antibiotics est cal igate the risk factors of infantile antibiotics associated diarrhea（AAD）, so as to provide nursing, 399 child-patients date who were admitted in author＇s hospital and had re- erapy,were retrospectively analysed and grouped according to whether AAD oceured, then the risk factors effecting AAD were analysed.As results, among 399 cases, 91 were suffered from AAD （22.8%）.Single factor analysis showed AAD development was related to infantile age,severe condition de- gree, hemoglobin level, antibiotics medication time, combined drug＇s kinds, hormone usage, microeco-agent use,invasive manipulation,and hospital stay time, significantly（ P 〈0.05）; Logistic regression analysis showed severe condition, long-term usage of antibiotics, the kinds of combined drugs 92, used, microeco-a- gents, with invasive manipulation were the independent related factors to AAD（ P〈0.05）,in these, using microeco-agents was a protective factor（ OR〈l）.Results show that the risk factors such as severe condi- tion,long term antibiotics use, multi-drug combination use, and invasive manipulation results in infantile AAD becoming high incidence, so clinically it should be rationally using antibiotics,and prophylactic using microeco-agems to prevent from AAD.
