长期家庭氧疗联合雾化吸入治疗COPD的疗效观察

Long-term home oxygen therapy combined with aerosol inhalation of budesonide for COPD

目的 观察评价长期家庭氧疗联合布地奈德雾化吸入治疗慢性阻塞性肺病（COPD）的临床效果.方法 将86例稳定期COPD患者随机对照试验分为两组,对照组43例给予常规对症疗法治疗,观察组43例在常规对症疗法基础上给予长期家庭氧疗联合布地奈德雾化吸入治疗,并比较两组的临床效果.结果 观察组总有效率为93.02％,明显高于对照组的76.74％（P＜0.05）;治疗后,观察组与对照组FEV1较治疗前均明显升高（均P＜0.05）,观察组FEV1明显高于对照组（P＜0.05）;治疗后,观察组与对照组FVC较治疗前均显著升高（均P＜0.05）,观察组FVC显著高于对照组（P＜0.05）;治疗后,观察组与对照组FEV1/FVC较治疗前均显著升高（均P＜0.05）,观察组FEV1/FVC显著高于对照组（P＜0.05）;治疗后,观察组气促消失时间、咳嗽消失时间、肺部啰音消失时间均显著低于对照组（均P＜ 0.05）.结论 长期家庭氧疗联合布地奈德雾化吸入治疗COPD能够提高临床疗效,增加组织供氧,改善患者的呼吸功能,延缓病情发展,并提高患者的生活质量.

Objective To evaluate the clinical effect of long-term home oxygen therapy combined with aerosol inhalation of budesonide in the treatment of chronic obstructive pulmonary disease （COPD）.Methods Eighty-six patients with stable COPD were randomly divided into a control group and an observation group,43 for each group.The control group were given conventional symptomatic therapy;in addition,the observation group were given long-term home oxygen therapy and aerosol inhalation of budesonide.The clinical effects were compared between the two groups.Results The total effective rate was 93.02% in the observation group and 76.74% in the control group （P〈0.05）.The FEV1,FVC,and FEV1/FVC were significantly higher after than before the treatment in both groups （all P〈0.05）,and were so in the observation group than in the control group after the treatment （all P〈0.05）.The times for disappearance of shortness of breath,cough disappearance,and lung arpeggios were significantly shorter in the observation group than in the control group （all P 〈0.05）.Conclusions Long-term home oxygen therapy combined with aerosol inhalation of budesonide for chronic obstructive pulmonary disease can improve clinical efficacy,tissue oxygen supply,patients＇ respiratory function and quality of life,and disease progression.
