两种静脉输液途径应用于极低出生体质量儿中的效果及成本比较

Comparison of effects and cost of two intravenous infusion routes in very low birth weight infants

目的探讨两种静脉输液途径应用于极低出生体质量儿中的效果及成本比较。方法选取2014年6月～2015年6月该院收治的极低出生体质量患儿80例作为对照组，再选取2015年7月～2016年7月该院收治的极低出生体质量患儿80例作为观察组。对照组行外周静脉短导管输液（PIV），观察组行经外周静脉置人中心静脉导管输液（PICC），比较两组输液效果和成本。结果观察组第1周、2周、3周、4周体质量增长均高于对照组，差异有统计学意义（均P〈0．05）。观察组并发症发生率低于对照组，差异有统计学意义（P〈0．05）。观察组单次置管成本高于对照组，差异有统计学意义（P〈0．05）；观察组月成本与对照组接近，差异无统计学意义（P〉0．05）。观察组成本一效果比低于对照组，差异有统计学意义（P〈0．05）。结论PICC相比于PIV应用于极低出生体质量儿中效果显著，能有效促进体质量增长，降低并发症发生率，减少月成本，提高成本一效果比，值得推广。

Objective To explore the effect and cost comparison of two intravenous infusion routes in very low birth weight infants. Methods A total of 80 cases of children with very low birth weight from June 2014 to June 2015 were selected as the control group, and 80 children patients with low birth weight from July 2015 to July 2016 were se- lected as the observation group. The control group was treated with peripheral venous catheter infusion （PIV）. The ob- servation group was treated with peripheral venous catheter infusion （PICC）. The transfusion effect and costs were compared. Results The body weight of the observation group was higher than that of the control group at first week, second week, third week and fourth week, and the differences were statistically significant （P〈0. 05）. The incidence of complications of the observation group was lower than that of the control group, and the difference was statistically significant （P〈0. 05）. The cost of single intubation in the observation group was higher than that of the control group and the difference was statistically significant （P〈0. 05）. The monthly cost of the observation group was similar to that of the control group, and the difference was not statistically significant （P〉0. 05 ）. The cost-effectiveness ratio of the observation group was lower than that of the control group, and the difference was statistically significant （ P〈0. 05 ）. Conclusion PICC is effective compared with PIV in extremely low birth weight which can effectively promote the growth of body weight, reduce the incidence of complications and the monthly cost and improve the cost-effectiveness ra-
