心理弹性的心理支持方案用于面部深度烧伤美容患者的临床研究

Clinical Application of Psychological Support Program Based on Resilience in Facial Deep Burn Patients

目的：构建基于心理弹性的心理支持方案,探讨该方案对面部深度烧伤美容患者的应用价值。方法：纳入2010年1月至2013年1月于我院接受治疗的71例面部深度烧伤美容患者为对照组,总结护理经验并制定基于心理弹性的心理支持方案。以2014年1月至2017年1月于我院接受治疗的同类患者68例为观察组,应用上述心理支持方案。分别于患者入院时、出院时测评心理弹性问卷（CD-RISC）、伤残接受度量表（AODS）、生活质量问卷（SF-36）得分变化情况。结果：两组患者创面均修复成功,两组手术时间、创面愈合时间、住院时间对比,差异均无统计学意义。出院时观察组坚韧与控制、力量、乐观得分均明显高于对照组,组间差异有统计学意义（t=16.392,8.698,7.714;P〈0.05）。CD-RISC评分与AODS、SF-36评分均呈明显正相关（r=0.711,0.689;P〈0.05）。结论：构建基于心理弹性的心理支持方案能够显著提升面部深度烧伤患者心理弹性,这可能有助于进而提升患者对自身伤残的接受度及生活质量。

Objective：To construct a psychological support program based on resilience,and explore the application value of this program for facial deep burn patients.Methods：A total of 71 patients with facial deep burn treated with cosmetic surgery in our hospital from January 2010 to January 2013 were enrolled as the control group,their nursing experience were summed up in order to construct a psychological support program based on resilience.A total of 68 patients with same condition treated in our hospital from January 2014 to January 2017 were enrolled as the observation group,they were nursed with the psychological support program.The changes in scores of The Connor-Davidson Resilience Scale（CD-RISC）,Acceptance of Disability Scale（AODS）and the MOS item short from health survey（SF-36）were measured at admission and discharge respectively.Results：The wound healing in both groups was successful.There was no significant difference in the operation time,wound healing time and hospital stay between the two groups.At discharge,the scores of strength and optimism in the observation group were significantly higher than those in the control group,with significant differences between the two groups（t=16.392,8.698,7.714;P〈0.05）.The CD-RISC score was positively correlated with the AODS and SF-36 scores（r=0.711,0.689;P〈0.05）.Conclusion：The psychological support program based on resilience can significantly improve the resilience of patients with deep facial burns,which may help to promote the acceptance of self disability and the quality of life.
