持续早期综合护理干预对妊娠期糖尿病患者围产期血糖及母儿结局的影响

Effect of comprehensive intervention on maternal blood glucose and infant outcomes in gestational diabetes mellitus

目的探讨持续早期综合护理干预对妊娠期糖尿病患者围产期血糖及母儿结局的影响。方法选取2015年9月至2016年9月本院收治的妊娠期糖尿病患者78例,随机分为观察组和对照组,每组39例。对照组患者在围产期早期给予妇产科常规护理干预,观察组在对照组的基础上施行综合性护理干预。记录两组患者干预前、干预2周后的血糖水平及母儿结局并进行组间比较。结果干预2周后,观察组患者餐后血糖水平明显低于对照组,高血压、羊水过多、剖宫产、酮症酸中毒等围产期并发症明显少于对照组,且新生儿发生肺炎、窒息、呼吸窘迫综合征等明显少于对照组,差异均有显著性(P<0.05)。结论综合性护理干预措施有利于控制妊娠期糖尿病患者围产期血糖水平,改善母儿结局,值得临床推广。

Objective To explore and analyze the effects of comprehensive interventions and the effects on blood glucose and maternal and infant outcomes in the early stages of pregnancy. Method 78 patients with gestational diabetes mellitus in our hospital were selected as the research object,and were randomly divided into two groups（n=39）. Group A were taken the routine nursing intervention of gynaecology and obstetrics, which group B patients with comprehensive nursing intervention implemented on the basis of routine nursing.To observe the blood glucose and maternal and infant outcomes in two groups. Result After two weeks of interventions, patients in the Group B had a lower blood glucose than those in the Group A. The incidence of hypertension, polyhydramnios, cesarean section and retoacidosis of patients and neonatal pneumonia, neonatal asphyxia and respiratory distress syndrome of infants was lower in Group B than that in Group A, the difference was statistically significant（P〈0.05）. Conclusion Comprehensive interventions taken early in the period of the pregnancy of gestational diabetes have positive implications for blood glucose control and maternal and infant outcomes.
