穴位电刺激复合麻醉对肝癌合并2型糖尿病患者术后转归的影响

Effect of acupoint electrical stimulation combined anesthesia on postoperative outcome of patients with hepatocellular carcinoma complicated with type 2 diabetes mellitus

目的探讨穴位电刺激复合麻醉对肝癌合并2型糖尿病患者术后转归的影响。方法选取2017年1月至2018年3月广东省珠海市第五人民医院收治的64例肝癌合并2型糖尿病患者作为研究对象,所有患者均行肝癌切除术治疗,按照不同麻醉方法将其分为对照组与观察组,各32例,对照组患者采用异丙酚全凭静脉麻醉,观察组患者采用穴位电刺激复合麻醉,比较两组患者的肝肾功能。结果两组患者术后24 h、72 h的胱抑素C、尿蛋白较麻醉诱导前均有明显变化,但观察组患者的变化幅度均小于对照组,差异均有统计学意义(均P<0.05)。两组患者术后3 d的天冬氨酸转氨酶(AST)、丙氨酸转移酶(ALT)、总胆红素(TBIL)较麻醉诱导前均明显提升,但观察组患者的AST、ALT提升幅度均小于对照组,差异均有统计学意义(均P<0.05)。结论穴位电刺激复合麻醉应用于肝癌合并2型糖尿病患者手术麻醉中效果显著,对患者肝肾功能的影响较小,有助于术后转归。

Objective To investigate the effect of acupoint electrical stimulation combined anesthesia on postoperative outcome of patients with type 2 diabetes mellitus. Methods 64 cases of liver cancer combined with type 2 diabetes treated in fifth people＇s Hospital of Zhuhai, Guangdong province from January 2017 to March 2018 were selected as the research object. All patients were treated with hepatectomy, According to different anesthesia methods, they were divided into control group and observation group 32 cases, and the control group was observed with propofol total intravenous anesthesia. The group received acupoint stimulation combined with anesthesia to compare the liver and kidney functions of the two groups. Results The two groups of patients with 24 h, 72 h, C and urine protein were significantly changed before anesthesia induction, but the changes in the observation group were less than those in the control group, and the difference was significant （P〈0.05）. The AST, ALT and TBIL of 3D in the two groups were significantly improved after anesthesia induction,but the amplitude of AST and ALT in the observation group was less than that of the control group, and the difference was significant （P〈0.05）. Conclusion the effect of acupoint electric stimulation combined with anaesthesia in the operation anesthesia of the patients with type 2 diabetes mellitus is significant. It has little effect on the function of liver and kidney, which is helpful to the postoperative outcome.
