摘要
目的探讨胃癌患者幽门螺杆菌(Hp)感染与核因子-κB(NF-κB)、热休克蛋白70(HSP70)、趋化因子L20(CCL20)的表达关系。方法选择2015年3月-2017年3月医院收集正常胃黏膜组织65份及胃癌组织73份、癌旁组织73份标本。所有标本均进行快速尿素酶试验和改良Gimesa染色判断Hp的感染情况。通过免疫组化链霉菌抗生物素蛋白-过氧化物酶连结法(S-P法)检测NF-κB、CCL20、HSP70的表达情况。比较癌旁组织、胃癌组织、正常胃黏膜的Hp感染率及NF-κB、CCL20、HSP70阳性率;分析NF-κB、CCL20、HSP70在癌旁组织、胃癌组织、正常胃黏膜中的表达灰度值。分析Hp感染对NF-κB、CCL20、HSP70表达的影响。对NF-κB、CCL20、HSP70的表达分进行Pearson相关性分析。结果胃癌组织和癌旁组织的Hp感染阳性率及NF-κB、CCL20、HSP70阳性率分别为43.84%及64.38%、75.34%、87.67%和47.95%及39.73%、43.84%、56.16%均高于正常胃黏膜;胃癌组织中NF-κB、CCL20、HSP70的阳性率均高于癌旁组织(P<0.001);正常胃粘膜、癌旁组织、胃癌组织中NF-κB、CCL20、HSP70的免疫组化染色灰度值呈递减趋势(P<0.001)。Hp(+)的癌旁组织及胃癌组织中,NF-κB、CCL20、HSP70的表达灰度值分别为(121.32±12.69)、(112.20±10.47)、(115.50±10.65)和(152.65±16.33)、(149.52±12.30)、(160.64±11.47)均低于Hp(-)组(P<0.001)。NF-κB、CCL20、HSP70间两两呈正相关(P<0.001)。结论 Hp感染是引起胃癌的重要危险因素,在胃癌及癌旁组织中存在较高水平的NF-κB、CCL20、HSP70,可能共同参与了胃癌的发生发展过程。
OBJECTIVE To explore the relationship between Helicobacter pylori infection and NF-κB,CCL20 and HSP70 expression in patients with gastric cancer.METHODS From Mar. 2015 to Mar 2017,211 samples were collected,including 65 samples of normal gastric mucosa,73 samples of gastric cancer tissues and 73 adjacent tissue samples.All specimens underwent rapid urease test and improved Gimesa staining to determine Hp infection.The expression of NF-κB,CCL20 and HSP70 was detected by S-P method.The positive rates of Hp infection and NF-κB,CCL20 and HSP70 were compared between cancer adjacent tissues,gastric cancer tissues,and normal gastric mucosa.Statistical analysis was performed to analyze the expression of NF-κB,CCL20,HSP70 in cancer adjacent tissues,gastric cancer tissues and normal gastric mucosa.The effect of Hp infection on expression of NF-kB,CCL20 and HSP70 was analyzed.The expression of NF-κB,CCL20 and HSP70 was analyzed by Pearson correlation analysis.RESULTS The positive rates of Hp infection and NF-κB,CCL20 and HSP70 in gastric cancer tissues and cancer adjacent tissues were 43.84%,64.38%,75.34%,87.67%and 47.95%,39.73%,43.84%,56.16%,respectively,which were all significantly higher than those of normal gastric mucosa.The positive rates of NF-κB,CCL20 and HSP70 in gastric cancer tissues were significantly higher than those of the adjacent tissues(P〈0.001).In normal gastric mucosa,adjacent tissues and gastric cancer tissues,immunohistochemical staining gray value of NF-κB,CCL20 and HSP70 showed a decreasing trend significant(P〈0.001).In Hp(+)gastric cancer tissues and adjacent tissues,immunohistochemical staining gray values of NF-κB,CCL20 and HSP70 were(121.32±12.69),(112.20±10.47),(115.50±10.65)and(152.65±16.33),(149.52±12.30),(160.64±11.47),which were all significantly lower than those of Hp(-)group(P〈0.01).There was a significant positive correlation between NF-κB,CCL20 and HSP70(P〈0.001).CONCLUSION HP infection is an important risk factor for gastric cancer.In gastric cancer and adjacent tissues,there are high levels of NF-κB,CCL20 and HSP70,which may be involved in the development of gastric cancer.
