外科手术患者术后切口感染相关因素分析

Related factors for postoperative incision infection in patients undergoing surgical procedures

目的探讨外科手术患者术后切口感染的相关因素。方法回顾性分析2014年6月-2017年6月于医院外科行手术治疗的患者2 324例的临床资料,观察患者术后切口及引流管相关感染情况;对患者年龄、性别、手术部位、手术类型、疾病类型、术后有无引流管放置等临床资料进行分析,归纳外科手术患者术后切口感染的相关因素。结果 2 324例行外科手术患者术后发生切口感染率为1.59%(37/2 324),其中切口感染37例,31例浅部组织感染、6例深部组织感染;428例放置引流管患者术后22例发生引流管相关性感染,感染率为5.14%;术后引流管相关性感染中15例引流管病原菌定植、6例引流管创口感染、1例引流管部位深部组织感染;年龄、手术部位、Ⅲ类手术、恶性疾病、合并糖尿病是外科手术患者术后切口感染的相关因素(P<0.05)。结论外科手术患者术后切口及引流管相关性感染率相对较低,切口感染相关因素较多,应针对相关因素进行干预以降低术后切口感染率。

OBJECTIVE To explore the related factors for postoperative incision infection in patients undergoing surgical procedures.METHODS The clinical data of 2 324 patients who received surgical procedures from Jun 2014 to Jun 2017 were retrospectively analyzed,the incidence of postoperative incision infection and drainage tube-related infection was observed.The clinical data of the enrolled patients,including age,genders,surgical sites,types of surgery,types of disease and postoperative drainage tube indwelling,were analyzed,and the related factors for the postoperative incision infection were summarized.RESULTS Among the 2 324 patients undergoing the surgical procedures,the incidence rate of postoperative incision infection was 1.59%（37/2 324）,37 had incision infection,31 had superficial tissue infection,and 6 had deep tissue infection.Of 428 patients who were treated with postoperative drainage tube indwelling,22 had drainage tube-related infection,with the infection rate 5.14%.Among the patients with postoperative drainage tube-related infection,15 had the colonization of pathogens on drainage tube,6 had drainage tube wound infection,and 1 had deep tissue infection at drainage tube indwelling site.The age,surgical sites,type III surgery,malignancies and complication with diabetes mellitus were the related factors for the postoperative incision infection in the patients undergoing the surgical procedures（P〈0.05）.CONCLUSIONThe incidence of postoperative incision infection and drainage tube-related infection is relatively low among the patients undergoing the surgical procedure.There are a variety of related factors for the incision infection,and it is necessary to take targeted intervention measures so as to reduce the incidence of the postoperative incision infection.
