腰椎旁肌脂肪浸润与椎间盘退变及年龄的相关性研究

Correlation of Paraspinal Muscles Fatty Infiltration and Lumbar Intervertebral Disc Degeneration and Age

目的使用m-DIXON Quant技术定量评估椎旁肌脂肪含量,探讨腰椎旁肌脂肪含量与椎间盘退变及年龄的相关性。方法 105名志愿者行常规磁共振检查及m-DIXON Quant序列扫描,评估L（3/4）、L（4/5）、L5/S1水平椎间盘Pfirrmann分级及测量其相应水平双侧竖脊肌、多裂肌及腰大肌脂肪含量。分析椎旁肌脂肪含量与椎间盘退变及年龄的相关性。结果竖脊肌、多裂肌及腰大肌脂肪分数与年龄有相关性（均P=0.00）;不同Pfirrmann分级的竖脊肌、多裂肌及腰大肌脂肪含量有统计学差异（均P=0.00）;控制年龄因素后,双侧竖脊肌脂肪含量与腰椎间盘Pfirrmann分级有相关性（左侧r=0.15,P=0.01;右侧r=0.13,P=0.02）;双侧多裂肌脂肪含量与腰椎间盘Pfirrmann分级有相关性（左侧r=0.15,P=0.01,右侧r=0.14,P=0.01）;双侧腰大肌脂肪含量与腰椎间盘Pfirrmann分级无相关性（左侧r=0.07,P=0.25,右侧r=0.03,P=0.54）。结论竖脊肌、多裂肌及腰大肌脂肪浸润程度随着年龄增大而增加;竖脊肌、多裂肌脂肪浸润程度与腰椎间盘退变程度有相关性,但相关并不密切;腰大肌脂肪浸润程度与腰椎间盘退变程度无相关性。

Objective The purpose of this study was to quantitatively assess the fraction of fat of paraspinal muscle with m-DIXON Quant and its correlation with intervertebral disc degeneration and age. Methods 105 adult patient volunteers received regular MRI and m-DIXON Quant imaging. Fat faction of erector spinae,multifidus muscle and psoas major were measured bilaterally from fat fraction map at L（3/4）、L（4/5）、L5/S1 levels,and the discs were assessed by Pfirrmann Grades. Statistical correlation analysis of fat fraction of paraspinal muscle with age and that with Pfirrmann Grade were performed. Results Fat faction of erector spinae,multifidus muscle and psoas major were all significantly associated with age（ P =0. 00）. And it showed significant difference between Pfirrmann Grades（ P = 0. 00）. After control age,Fat faction of erector spinae and multifidus muscle were associated with Pfirrmann Grades（ ES： L r = 0. 15,P = 0. 01; R r = 0. 13,P = 0. 02;MS： L r = 0. 15,P = 0. 01,R r = 0. 14,P = 0. 01）,while that of psoas major were showed uncorrelated with Pfirrmann Grades（ L r = 0. 07,P = 0. 25,R r = 0. 03,P = 0. 54）. Conclusion Fatty Infiltration of paraspinal increased with age.Fatty Infiltration of erector spinae and multifidus muscle were associated with degeneration of intervertebral disc,when that was not of psoas major.
