吸入用复方异丙托溴铵溶液对AECOPD肺部通气功能指标、血气分析指标及QLI评分的影响 被引量:10

摘要 目的探究吸入用复方异丙托溴铵溶液对慢性阻塞性肺疾病急性加重(acuteexacerbationofchronic obstructivepulmonarydisease,AECOPD)患者肺部通气功能指标、血气分析指标及QLI评分的影响。方法选取解放军第477医院呼吸内科2017年2月—2018年3月收治的100例AECOPD患者,按照不同治疗方案分为观察组(50例)和对照组(50例),对照组采用常规呼吸治疗,观察组采用常规呼吸治疗的基础上加用复方异丙托溴铵(吸入),观察2组患者肺部通气功能指标,血气指标以及QLI评分。结果观察组治疗1周后总有效率高于对照组,差异具有统计学意义(P<0.05)。2组患者治疗前用力肺活量(forced vital capacity, FVC)、第1 s用力呼气流量(forced expiratory volume in first,FEV1)、第1 s用力呼气量占用力肺活量百分比(forced expiratory volume in the first second to forced vital capacity,FEV1%)、QLI评分、二氧化碳分压(PaCO_2)、氧分压(PaO_2)差异无统计学意义(P>0.05),治疗12 h和1周后2组患者FVC、FEV1%、FEV1、PaCO_2、PaO_2均有所改善,且各指标观察组明显优于对照组,差异具有统计学意义(P<0.05)。QLI评分治疗后2组患者评分均提高,且观察组优于对照组(P<0.05)。结论 AECOPD患者吸入复方异丙托溴铵治疗效果显著,肺部功能指标显著提高,动脉血气指标得到改善,改善患者生活质量,疗效确切,值得临床推广。 ObjectiveTo investigate the effects of inhaled combivent on pulmonary ventilation function, blood gas indicators and QLI score in acute exacerbation of chronic obstructive pulmonary disease(AECOPD). MethodsOne hundred AECOPD patients treated in the Department of Respiration of our hospital between February 2017 and March 2018 were selected and divided into the observation group and control group according to treatment methods, with 50 cases in each. The control group received the routine treatment, whereas the observation group was additionally given inhaled combivent. Then, the pulmonary ventilation function, blood gas indicators and QLI score of the two groups were recorded. ResultsThe total effective rate in the observation group after one week of treatment was higher than that in the control group(P〈0.05). The FVC, FEV1%, FEV1, QLI score, PaCO2, PaO2 of the two groups were not significantly different before treatment(P〉0.05), but were significantly ameliorated at 12 h and 1 week after treatment, especially in the observation group(P〈0.05). The QLI score was significantly improved after treatment in both groups, and was better in the observation group than in the control group(P〈0.05). ConclusionThe inhaled combivent is an effective way to ameliorate the pulmonary ventilation function, blood gas, which can also improve the quality of life and is worthy of recommendation.
