COX-2基因多态性与Hp感染在食管癌发生发展过程中的作用 被引量:4

摘要 目的探讨环氧合酶-2(COX-2)基因多态性与幽门螺杆菌(Hp)感染在食管癌发生发展过程中的作用。方法选取2015年5月-2018年5月肥城市人民医院治疗的食管鳞癌患者114例(观察组),同时选取健康人群342例作为对照组,采用聚合酶链反应-限制性片段长度多态性检测COX2 765基因多态性,采用Giemsa染色检测Hp感染。结果观察组COX-2 765基因GC基因型和C等位基因比例分别为28.07%和14.04%,均明显高于对照组(P<0.05);观察组Hp感染率为17.54%,明显低于对照组(P<0.05);不同临床病理特征患者COX-2 765基因GC型比例比较差异无统计学意义(P>0.05);中低分化患者Hp感染率为2.63%,明显低于高分化患者(P<0.05); GC型食管癌患者Hp感染率为3.13%,明显高于GG型食管癌患者(P<0.05)。结论 COX-2 765基因多态性与食管癌遗传易感性有关,Hp感染与食管癌分化程度有关,COX-2 765基因多态性与Hp感染可能存在交互作用,值得进一步研究。 Objective To investigate the role of cyclooxygenase-2( COX-2) gene polymorphism in Helicobacter pylori( Hp) infection during the development and progression of esophageal cancer.Methods Between May 2015 and May 2018,114 cases of esophageal squamous cell carcinoma( observation group) were selected as the observation group,and another 342 healthy people were selected as the control group. Polymerase chain reaction restriction fragment length polymorphism was used to detect the polymorphism of COX-2 765 gene while Hp infection was detected by enzyme-linked immunosorbent assay. Results The GC genotype and C allele percentage of COX-2 765 gene in the observation group was 28.07% and 14.04% respectively,significantly higher than in the control group( P〈0.05). The infection rate of Hp in the observation group was 17.54%,which was significantly lower than that in the control group( P〈0. 05). There was no significant difference in the GC percentage of COX-2 765 gene between patients with different clinicopathological features( P〈0.05). The infection rate of Hp in moderately poorly differentiated patients was 2.63%,which was significantly lower than that in patients with high differentiation( P〈0.05).The infection rate of Hp in patients with type GC esophageal cancer was 3.13%,which was significantly lower than that in patients with type GG esophageal cancer( P〈0. 05). Conclusion COX-2 765 gene polymorphism may be associated with genetic susceptibility to esophageal cancer,while Hp infection may be related to the differentiation degree of esophageal carcinoma.COX-2 765 gene polymorphism may interact with Hp infection,so it is worthy of further study.
