超声检查联合血清中β-HCG水平在宫外孕患者中的临床运用

Clinical Value of Serum HCG Combined with Ultrasonography in Ectopic Pregnancy

目的探讨超声检查联合血清中β-绒毛膜促性腺激素(β-HCG)在宫外孕患者中的临床运用。方法选取2015年9月-2017年9月在本院就诊疑似宫外孕的患者150例作为研究对象,所有患者均行经腹部和经阴道超声检查,并检测患者血清中β-HCG水平,探讨三者在宫外孕中的诊断价值。结果宫内妊娠的子宫内膜厚度(经腹部)、子宫内膜厚度(经阴道)、血清β-HCG水平明显高于宫外孕患者,三者联合运用对宫内妊娠和宫外妊娠的诊断准确率均为100%,且明显高于单独使用的诊断准确率,而超声检查联合血清β-HCG水平的诊断效能最高,其敏感性、特异性、阳性预测值、阴性预测值分别为87.9%、98.6%、79.8%、97.8%。结论超声检查联合血清中β-HCG水平检测有助于提高宫外孕的诊断准确率。

Objective To explore the clinical applicability of ultrasonography combined with serum beta-HCG in ectopic pregnancy patients.Methods One hundred and fifty cases of suspected ectopic pregnancy patients treated in our hospital between September 2015 and September 2017 were selected as subjects,all of whom underwent transabdominal and transvaginal ultrasonography. The beta-HCG levels in the serum of patients were detected and the value of the three approaches in the diagnosis of ectopic pregnancy was explored. Results The endometrial thickness（ the abdomen）,endometrial thickness（ vaginal）,and serum beta-HCG levels were significantly higher among patients of intrauterine pregnancy than those of patients with ectopic pregnancy.The accurate rate of combined diagnosis with these three approaches was 100% for intrauterine pregnancy and ectopic pregnancy,significantly higher than that of a single diagnosis. Ultrasonography,when combined with the serum beta-HCG level diagnosis,was the most sensitive and specific. The sensitivity,specificity,positive predictive value and negative predictive value were 87.9%,98.6%,98.5% and 97.8% respectively.Conclusion Detection with ultrasound combined with serum beta-HCG levels can help improve the diagnostic accuracy of ectopic pregnancy.
