单纯orthocord缝线修补冈上肌腱钙化性肌腱炎合并肩袖损伤的临床研究

Clinical Research of Using Pure Orthocord Stitches Suture Method for Calcified Tendinitis of the Supraspinatus Tendon Merge Injury of Rotator Cuff

目的:探讨采用单纯orthocord缝线修补冈上肌腱钙化性肌腱炎合并肩袖损伤的临床效果。方法:2013年1月-2017年1月,orthocord缝线修补组采用单纯orthocord缝线修补冈上肌腱钙化性肌腱炎合并肩袖损伤8例,缝合锚钉修补组用缝合锚钉修补冈上肌腱钙化性肌腱炎合并肩袖损伤12例。应用ASES功能评分及VAS疼痛评分,评估患者术前及术后1年随访时肩关节功能。结果:术后20例随访1年。orthocord缝线修补组ASES评分由术前的(48.8±8.7)分提高到术后的(83.6±5.3)分,VAS评分由术前的(7.7±1.2)分降低到术后的(2.2±1.1)分。缝合锚钉组ASES评分由术前的(47.8±8.3)分提高到术后的(84.2±4.7)分,VAS评分由术前的(7.4±1.3)分降低到术后的(2.1±1.2)分。两组患者患肢功能评估(ASES评分及VAS疼痛评分),术前与术后末次复查比较,评估差异有统计学意义(P<0.05),单纯orthocord缝线修补组术前及术后末次随访时评估与缝合锚钉组差异无统计学意义(P≥0.05)。两组患者均恢复良好,优良率达100%。结论:采用单纯orthocord缝线修补可作为冈上肌腱钙化性肌腱炎合并肩袖损伤的新的治疗策略,操作方便,疗效满意,并发症少。

Objective：To investigate use pure orthocord stitches suture method treat with calcified tendinitis of the supraspinatus tendon merge injury of rotator cuff.Method：From January 2013 to January 2017,8 cases of calcified tendinitis of the supraspinatus tendon merge injury of rotator cuff were repaired by use pure orthocord stitches suture method,and 12 cases of calcified tendinitis of the supraspinatus tendon merge injury of rotator cuff were repaired by suture anchors as control in our department.The patients were assessed by using American shoulder and elbow surgeons scores system and visual analogue scale system,from preoperative and follow-up（one year after operation）.Result：20 cases were followed up for 1 year.The patients repaired by use pure orthocord stitches suture method,ASES score from 48.8±8.7（preoperative） rising up to 83.6±5.3（one year after operation）,VAS score from 7.7±1.2（preoperative） reduced to 2.2±1.1（one year after operation）.The patients repaired by suture anchors method,ASES score from 47.8±8.3（preoperative） rising up to 84.2±4.7（one year after operation）,VAS score from 7.4±1.3（preoperative） reduced to 2.1±1.2（one year after operation）.There were significant differences between preoperative and one year after operation（P〈0.05）.But there were not significant differences between the patients repaired by use pure orthocord stitches suture method and the patients repaired by suture anchors method（P≥0.05）.Limb function recovered well postoperatively,the excellent and good rate was 100%.Conclusion：Use pure orthocord stitches suture method treat with calcified tendinitis of the supraspinatus tendon merge injury of rotator cuff is as a new treatment strategies,easy to operate,curative effect is satisfactory,fewer complications.
