早产儿呼吸窘迫综合征治疗中采用猪肺磷脂注射液的可行性分析

Feasibility Analysis of Use of Poractant Alfa Injection in the Treatment of Premature Infants with Respiratory Distress Syndrome

目的:探讨猪肺磷脂注射液(固尔苏)在早产儿呼吸窘迫综合征(NRDS)治疗中的应用效果,评估可行性。方法:取2015年7月-2017年11月在笔者所在医院分娩的38例NRDS患儿,所有病例均给予固尔苏,比较患儿治疗前后肺氧合功能、机械通气参数变化情况。结果:给药治疗后,患者临床症状均有所改善,用药后2、6、12 h患儿动脉血PaO2、pH与PaCO2指标监测结果和治疗前比较,差异均有统计学意义(P<0.05);上述时间点,患儿机械通气参数FiO2、PIP、PEEP、RP与用药治疗前,均显著降低,差异有统计学意义(P<0.05);本次研究中无患儿死亡,也未出现严重并发症。结论:应用固尔苏治疗NRDS,有助于改善患儿肺氧合功能,显著降低机械通气参数与病死率,可在本病症临床治疗中进一步推广。

Objective：To explore the application effect of Poractant Alfa Injection（Curosurf） in the treatment of premature infants with respiratory distress syndrome（NRDS） and evaluate the feasibility.Method：38 children with NRDS in our hospital from July 2015 to November 2017 were selected.All the cases were given Curosurf.The changes of pulmonary oxygenation function and mechanical ventilation parameters were compared before and after treatment.Result：After the treatment,the clinical symptoms of the patients were improved.The data of PaO2,pH and PaCO2 in 2 h,6 h and 12 h were statistically significant different compared with those before treatment（P〈0.05）.The parameters of mechanical ventilation were FiO2,PIP,PEEP and RP were all decreased compared with before the treatment,the difference was statistically significant（P〈0.05）.No case died and no serious complications occurred in this study.Conclusion：The treatment of Curosurf for NRDS is helpful to improve the oxygenation function of the lung,reduce the mechanical ventilation parameters and the mortality,and can be further promoted in the clinical treatment of this disease.
