补金逐水汤治疗恶性胸腔积液的临床效果观察

Clinical Observation of Bujin Zhushui Decoction in Treating Malignant Pleural Effusion

目的:评价补金逐水汤在恶性胸腔积液中的临床应用效果。方法:选择2015年11月-2018年8月于南京市胸科医院急诊科住院的恶性胸腔积液患者40例,按随机数字表法分为对照组和研究组各20例,对照组予胸腔闭式引流联合胸腔灌注顺铂治疗,治疗组在对照组治疗的基础上加予补金逐水汤治疗。比较两组患者在胸腔积液量、生活质量KPS评分及不良反应的差异。结果:治疗后,研究组患者再发胸腔积液情况明显改善,优于对照组患者,差异有统计学意义(P<0.05)。治疗前,两组患者KPS评分比较差异无统计学意义(P>0.05);治疗后,两组KPS评分均有提高,且研究组患者KPS评分高于对照组,差异有统计学意义(P<0.05)。对照组总不良反应率为85.0%,研究组为70.0%,差异有统计学意义(P<0.05)。结论:补金逐水汤治疗恶性胸腔积液,可减少胸腔积液量,减缓癌症的发展,改善患者生活质量,减少不良反应。

Objective：To evaluate the clinical application of Bujin Zhushui Decoction in malignant pleural effusion.Method：Forty patients with malignant pleural effusion during the hospitalization of the emergency department of Nanjing Chest Hospital from November 2015 to August 2018 were selected.The patients were divided into the control group and the study group by 20 patients according to the random number table.Thoracic closed drainage combined with thoracic perfusion cisplatin treatment, the treatment group was treated with Bujin Zhuishui Decoction on the basis of the treatment of the control group. The differences in pleural effusion volume,quality of life KPS score, and adverse reactions were compared between the two groups.Result： After treatment, the recurrent pleural effusion in the study group was significantly improved than that in the control group （P〈0.05）.Before treatment,there was no significant difference in KPS scores between the two groups （P〉0.05）,and after treatment,the KPS scores of the two groups were improved, and the KPS scores of the patients in the study group were higher than those in the control group （P〈0.05）.The total adverse reaction rate of the control group was 85.0% and that of the study group was 70.0%.The difference was statistically significant （P〈0.05）.Conclusion： Bujin Zhushui Decoction can reduce the amount of pleural effusion, reduce the development of cancer, improve the quality of life of patients, and reduce the occurrence of adverse reactions during the treatment.
