门诊不合理处方分析

Analysis of Unreasonable Prescription in Outpatient Department

目的分析门诊的不合理处方,提高处方质量,促进合理用药。方法随机抽取我院2018年1—4月的1 400张处方资料,对这些资料进行回顾性分析与整理,参照《处方管理办法》、药品说明书等进行处方点评,进行合理性的分析。结果 1 400张处方中不合理的有114张(8.14%),不合理处方的类别主要表现在处方临床诊断不全、药品的用法用量不适宜、药品的适应证与诊断不符、单张处方超出5种药物、用量不明确以及无特殊情况下处方超量等方面。结论我院的门诊处不合理处方有待进一步提高,通过分析,进一步提高用药。

Objective To analyze the irrational prescriptions of outpatients, improve the quality of prescriptions and promote the rational use of drugs. Methods 1 400 prescription data from January to April 2018 in our hospital were randomly selected. These data were retrospectively analyzed and collated. Prescription reviews were consulted with reference to the Prescription Management Measures and drug descriptions, and rational analysis was conducted. Results There were 114 unreasonable prescriptions （8.14%） in 1 400 prescriptions. The category of unreasonable prescriptions was mainly manifested in incomplete clinical diagnosis of prescriptions, inappropriate usage and dosage of medicines, inapplicability of medicines and diagnosis, and single prescriptions exceeding 5 drugs, the dosage was not clear, and there was no prescription overdose in special circumstances. Conclusion The unreasonable prescription in the outpatient department of our hospital needs further improvement. Through analysis, the use of drugs can be further improved.
