前路及后路手术治疗胸腰椎多节段脊柱骨折的临床效果对比

Comparison of clinical effects of anterior and posterior surgeries in treatment of multi-segmental thoracolumbar spine fractures

目的:比较前路及后路手术治疗胸腰椎多节段脊柱骨折的效果。方法:选取胸腰椎多节段脊柱骨折患者76例作为观察对象。采用随机数字表法将其分为对照组和观察组各38例。观察组接受前路手术,对照组接受后路手术。对比两组手术前后椎体前后缘高度、脊椎融合率和并发症发生率。结果:观察组手术时间、术中出血量及住院时间均低于对照组(P<0.05);观察组术后即刻、术后8个月椎体前缘高度、椎体后缘高度均明显高于对照组;观察组脊椎融合率为100.00%,明显高于对照组的81.58%;观察组并发症发生率为5.26%,明显低于对照组的26.32%,差异均有统计学意义(P<0.05)。结论:与后路手术相比,前路手术治疗胸腰椎多节段脊柱骨折,可明显提高患者的椎体前缘高度、后缘高度和脊椎融合率,且并发症少。

Objective： To compaye effects of anterior and posterior surgeries in treatment of multi-segmental thoracolumbay spine fractures. Methods： 76 patients with multi-segmental thoracolumbar spine fractures were selected as observation objects. They were divided into control group （n 38） and observation group （n 38） by the random number table method. The observation group received the anterior surgery, while the control group received the posterior surgery. The heights of the anterior and posterior vertebral body, the rate of spinal fusion and the incidence of complications were compared between the two groups before and after the surgery. Results： The operation time, intraoperative blood loss and hospitalization time of the observation group were lower than those of the control group （P〈0.05）. The height of the anterior vertebral body and the height of the posterior vertebral body were significantly higher in the observation group than in the control group. The spinal fusion rate of the observation group was 100.00%, which was significantly higher than that of the control group （81.58%）. The incidence of complications in the observation group was 5.26%, which was significantly lower than that in the control group （26.32%）, and the difference was statistically significant （P〈0.05）. Conclusions： Compared with the posterior surgery, the anterior surgery in the treatment of the multi-segmental thoracolumbar spine fracture is more effective, and can significantly improve the anterior and posterior vertebral body heights and spinal fusion rate with fewer complications.
