寻常型银屑病患者血清脂联素和抵抗素测定及与胰岛素抵抗相关性分析

Serum Levels of Adiponectin and Resistin and Their Correlation with Insulin Resistance in Patients with Psoriasis Vulgaris

目的分析寻常型银屑病(PV)患者血清脂联素和抵抗素水平及与胰岛素抵抗的相关性。方法纳入PV患者70例和正常对照40例,采用酶联免疫吸附试验(ELISA)方法测定血清脂联素和抵抗素水平,同时采用葡萄糖氧化酶法及化学发光法测定空腹血糖和胰岛素水平,并计算胰岛素抵抗指数(HOMA-IR)。结果 PV患者血清脂联素水平较对照组降低(P<0.05),而血清抵抗素水平高于对照组(P<0.05)。PV患者胰岛素抵抗指数高于对照组[2.54(0.62～28.71)vs. 2.01(0.56～6.39),P<0.05],其与脂联素呈负相关(r=-0.211,P<0.05),与抵抗素、银屑病皮损面积和严重程度指数(PASI)及病程无相关性(r=0.157、r=-0.095、r=0.093,P>0.05)。结论 PV患者易于发生胰岛素抵抗,脂联素可能在评估银屑病合并代谢性疾病中发挥作用。

Objective In order to analyze the correlation of serum adiponectin and resistin levels with insulin resistance in patients with psoriasis vulgaris（PV）. Methods Serum adiponectin and resistin levels in 70 PV patients and 40 control subjects were measured by ELISA. At the same time,fasting blood glucose and insulin levels were determined by glucose oxidase and chemiluminescence methods,respectively. Insulin resistance index was calculated using the steady state model of the homeostasis model assessment. Results Compared with the normal controls,serum adiponectin level decreased （P〈0.05） but serum resistin level increased in PV patients （P〈0.05）. HOMA-IR [2.54（0.62-28.71） vs. 2.01（0.56-6.39）,P〈0.05] was higher in patients with psoriasis vulgaris than in controls. There was a negative correlation between adiponectin and HOMA-IR（r=-0.211,P〈0.05）,but no correlation was found between resistin,psoriasis area and severity index,duration and the homeostasis model assessment （HOMA）-IR（r=0.157,r=-0.095,r=0.093,P〉0.05）. Conclusion Patients with psoriasis vulgaris are more insulin resistant compared with healthy control subjects. Serum level of adiponectin might be useful in evaluating psoriasis and its comorbidities.
