摘要
目的以兔耳郭后动脉和耳后静脉内侧支为供血动脉和回流静脉的岛状皮瓣(简称兔耳岛状皮瓣)为基础,建立透明质酸动脉栓塞皮肤缺血模型。方法采用新西兰大白兔8只(16只兔耳),制作以耳中央动静脉为主要血供的岛状皮瓣(面积为6.0 cm×1.5 cm)。将兔耳岛状皮瓣随机分为对照组和实验组。对照组只完成岛状皮瓣,不注射透明质酸;实验组完成岛状皮瓣后,从耳郭后动脉远端向耳郭后动脉内注射透明质酸40μl,分别于注射后即刻,1、7、15 d观察皮肤颜色、皮瓣水肿程度、微循环变化和栓塞皮瓣面积变化等情况。结果实验组皮瓣均出现不同程度的缺血坏死,对照组则未出现皮瓣坏死,且全部成活。结论以兔耳郭后动脉和耳后静脉内侧支为供血动脉和兔耳岛状皮瓣为基础,从耳郭后动脉(中央耳动脉)远端注射透明质酸,可以成功地制作出透明质酸动脉栓塞模型。
Objective Based on an island flap with the posterior auricular artery and the medial branch of the posterior auricular vein as blood supply and reflux vein, a skin ischemia model of hyaluronic acid(HA) arterial embolization was created. Methods Eight New Zealand white rabbits(16 ears) were used to establish a model with an islanded skin flap with the auricular central artery and vein as the main blood supply(6.0 cm × 1.5 cm). The rabbit ear island skin flaps were randomly divided into control and experimental groups. In the control group, the auricular island flaps were created without hyaluronic acid injection. In the experimental group, 40 μl hyaluronic acid was injected into the posterior auricular artery from the distal end. The skin color, degree of flap edema, microcirculation changes, and the area of skin necrosis were examined immediately and at 1, 7, and 15 days after injection. Results There were different degrees of skin necrosis in the experimental group, and no skin necrosis occurred in control group. Conclusion Using the rabbit auricular island flap with the posterior auricular artery and the medial branch of the posterior auricular vein as blood supply, a hyaluronic acid embolism model can be successfully created through hyaluronic acid injection into the posterior auricular artery(central auricular artery) from the distal end.
