摘要
目的:探讨丙泊酚全麻对家兔腹腔内高压动物模型腹内压的影响。方法:选取健康雄性家兔60只,体质量4~4. 5 kg,采用随机数字表法分为丙泊酚全麻组(Ⅰ组)和利多卡因局麻组(Ⅱ组)。Ⅰ组选家兔耳缘静脉给药,诱导用药丙泊酚6 mg/kg,静脉注射,维持用药38 mg·kg-1·h-1静脉持续泵注。Ⅱ组以0. 5%盐酸利多卡因局部浸润麻醉。常规皮肤准备、消毒、铺巾,逐层切开腹壁进入腹腔,将测压装置置于上腹部腹腔内,关闭腹腔,监测腹腔内压。Ⅰ组持续麻醉下监测腹腔内压,即麻醉的维持期P1,测压结束后,停止麻醉药物。距P1测压结束后10 min第二次测压即麻醉苏醒期P2。距P1测压后20 min第三次测压即麻醉清醒期P3。局部麻醉组以时间为节点与Ⅰ组相对应。两组分别于麻醉手术结束后抽静脉血3 m L,监测血清等指标进行比较。结果:Ⅰ组P1、P2腹腔内压与Ⅱ组P1、P2比较,腹腔内压降低,差异有统计学意义(P <0. 05),Ⅰ组P3腹腔内压与Ⅱ组P3比较,差异无统计学意义(P> 0. 05)。Ⅰ组与Ⅱ组胰岛素、白细胞介素-10(IL-10)、白细胞介素-8(IL-8)比较差异无统计学意义(P> 0. 05)。Ⅰ组血管紧张素Ⅱ低于Ⅱ组,多巴胺高于Ⅱ组,差异均有统计学意义(P <0. 05)。结论:丙泊酚全麻可使家兔腹腔内压降低,使血管紧张素Ⅱ降低,多巴胺升高,对胰岛素、IL-10、IL-8的影响,差异无统计学意义。
Objective :To study on rabbit model of propofol in intra -abdonfinal hypertension animal, intra -abdonfinal pressure effect. Methods : A total of 60 healthy male rabbits, weighing 4 - 4.5 kg, were randomly divided into 2 groups ( n = 30), propofol general anesthesia group (Group Ⅰ ) and lidoeaine local anesthesia group (Group Ⅱ ). Group Ⅰ selected rabbit ear vein adnfinistration of propofol induced drug 6mg after intravenous injection of kg -1, to maintain the continuous infusion of drug 38 mg · kg -1· h -1. Group Ⅱ received local infiltration anesthesia with 0.5% lidoeaine hydroehloride. Routine skin preparation, disilffeetion and towel laying were used to cut abdonfinal wall abdonfinal eavily. The pressure measuring device was placed in the upper abdonfinal cavity,and the abdonfinal eavily was closed to monitor the intra - abdonfinal pressure. Group Ⅰ continuous anesthesia monitoring intra - abdonfinal pressure,that is, the maintenance of anesthesia, P1 pressure after the end of the anesthetic stopped,P1 pressure after the end of the second 10 min pressure measurement, namely anesthesia recovery period P2- After P1 pressure measurement,the third 20 min pressure measurement was P3 during the anesthesia awake period,and the local anesthesia group was corresponding to the group Ⅰ with time as the node. The two groups were compared with venous blood 3ml monitoring serum after the end of anesthesia operation. Results:Compared with group P1 and P2 ,the intra -abdominal pressure of group Ⅰ , P2 and group P1 decreased, the difference was statistically significant ( P 〈 0.05 ). There was no significant difference in P3 between group Ⅰ and group Ⅱ ( P 〉 0.05 ). There was no significant difference in serum insulin, IL - 10,IL - 8 between group Ⅰ and group Ⅱ (P 〉 0.05). Angiotensin Ⅱ group was lower than group Ⅱ ,dopamine group Ⅰ was higher than that of group Ⅱ , the difference was statistically significant ( P 〈 0.05 ). Conclusion : General anesthesia with propofol can reduce the intra -abdominal pressure in rabbits, and propofol can reduce angiotensin Ⅱ and increase dopamine level. There was no significant difference in the effects of propofol on insulin, IL -10, IL -8.
