口服黄体酮治疗黄体功能不全所致先兆流产的临床分析

Clinical Analysis of Oral Progesterone in the Treatment of Threatened Abortion Caused by Luteal Insufficiency

目的分析口服黄体酮对黄体功能不全(Luteal phase defect,LPD)所导致的先兆流产的临床疗效。方法方便择取2016年2月—2018年2月该所诊治的64例LPD所致的先兆流产患者,按照入院顺序将其顺次编号,再应用随机数表将其分为对照组、研究组,各32例,对照组给予保胎灵(口服),研究组给予黄体酮(口服);比较两组临床症状消失时间,统计两组保胎成功率及不良反应率。结果研究组患者阴道流血、下腹疼痛消失时间分别为(8.12±1.14)d、(7.05±1.08)d,均短于对照组的(12.27±3.10)d、(11.77±2.95)d,差异有统计学意义(t=7.108,8.499,P<0.001);此外,研究组保胎成功率为96.88%,明显高于对照组81.25%,差异有统计学意义(χ~2=4.010,P=0.045);研究组不良反应率为6.25%,明显低于对照组25.00%,差异有统计学意义(χ~2=4.267,P=0.039)。结论口服黄体酮治疗LPD所导致的先兆流产具有疗效好、见效快、安全性高等优点,值得临床推广应用。

Objective To analyze the clinical efficacy of oral progesterone on threatened abortion caused by Luteal phase defect （LPD）. Methods The patients with threatened abortion caused by 64 cases of LPD diagnosed from February 2016 to Februry 2018 were randomly selected according to the order of admission, and then divided into the control group and the research group by random number table. In each of 32 cases, the control group was given Baotailing （oral）, and the study group was given progesterone （oral）; the time of disappearance of clinical symptoms was compared between the two groups, and the success rate and adverse reaction rate of the two groups were counted. Results The disappearance time of vaginal bleeding and lower abdominal pain in the study group was （8.12±1.14）d and （7.05±1.08）d, respectively, which were shorter than that of the control group （12.27±3.10）d and （11.77±2.95）d, the difference was statistically signfieant（t=7.108,8.499, P〈 0.001）; In addition, the success rate of the study group was 96.88%, which was significantly higher than that of the control group of 81.25%,the difference was statistically signfieant （X^2=4.010, P=0.045）; the adverse reaction rate of the study group was 6.25%, which was significantly lower in the control group, 25.00% , the difference was statistically signfieant （X^2=4.267, P=0.039）. Conclusion The treatment of LPG caused by oral progesterone has the advantages of good curative effect, quick effect and high safety. It is worthy of clinical application.
